Emergency Department
Tianjin First Central Hospital
The Emergency Department of Tianjin First Central Hospital was established in 1983. It has a certain scale, advanced equipment and a complete talent echelon configuration, and occupies a certain academic status in the professional field of emergency medicine in Tianjin. Emergency personnel focus on "patient interests" and adhere to the cultural concept of "focusing on academics, emphasizing integrity, strengthening skills, and cultivating benevolence". With the help of the hospital resource platform, the emergency department-led MDT diagnosis and treatment model for critically ill patients is realized. The emergency department and pre-hospital emergency care are seamlessly connected, and the emergency treatment channel for critical diseases such as chest pain, stroke, poisoning, high-risk pregnant women, trauma and dangerous upper gastrointestinal bleeding is unblocked, and the construction of the country's "major emergency, major emergency, and large platform" is promoted, and the concept of "people first, life first" is always practiced. Realize the modern emergency management concept of zero channel, short delay and high technology, promote the construction of ground-air emergency system, build the department into one of the largest emergency medicine centers in North China, and achieve the grand goal of becoming a "global medical leader" as a central hospital. In recent years, in accordance with the Tianjin Municipal Health Commission's 1+6 emergency system construction, a chest pain center, stroke center, trauma center, critical maternal and newborn treatment center have been established, and it has taken the lead in establishing a fast-track regional center for emergency treatment of dangerous upper gastrointestinal bleeding in Tianjin. We have rich experience in the diagnosis and treatment of multiple trauma, acute myocardial infarction and other high-risk chest pain, acute ischemic stroke, sepsis, dangerous upper gastrointestinal bleeding, high abdominal pain, and the treatment of elderly patients with multiple organ dysfunction and other acute and critical patients. With the improvement of the overall strength of Tianjin First Central Hospital, the medical team of the Emergency Medicine Department has also made great progress and development in discipline construction, overall medical staff management, medical quality, and teaching and research in accordance with the requirements of the Health Commission's "Emergency Department Construction and Management Guidelines". In recent years, with the adjustment of the layout of the city's medical centers, the intensification of traffic congestion, the progress of social welfare undertakings, the increase in the proportion of the elderly population, and the increase in the migrant population, the current annual number of medical visits exceeds 180,000, and the number of rescued patients exceeds 10,000. Among them, the proportion of patients with multiple trauma and elderly patients with multiple organ dysfunction has increased significantly. In order to improve medical quality, ensure medical safety, and improve work efficiency, the "Emergency Department Physician's Manual", "Emergency Nursing Quality Control Standards Manual" and "Emergency Nurse Operation Manual" are formulated (all updated every two years). The content of the manual includes the core system of the emergency department, diagnosis and treatment routines for common emergency diseases and multiple diseases, service procedures, and nursing operation procedures, etc. In order to better respond to high-incidence and highly fatal diseases and reduce disease mortality, relevant sub-specialty treatment groups have been established within the department to provide technical support for further specialist treatment and improvement of patient prognosis. In accordance with national requirements, we strictly implement the diagnosis and treatment specifications for corresponding diseases, and successfully passed the inspection, and were successively listed as "Chest Pain Center", "Stroke Center", "Fast Track for Emergency Treatment of Dangerous Upper Gastrointestinal Bleeding", "Trauma Center", etc. Under the normalized prevention and control of the COVID-19 epidemic, while our emergency department personnel are actively fighting the epidemic, we have revised the contingency plan for COVID-19 prevention and control in the Department of Emergency Medicine and actively conducted case reviews, desktop deductions, and on-site mobilizations to further strengthen the treatment of critically ill patients. For elderly patients with multiple organ dysfunction who are staying in the emergency department due to various reasons, individualized treatment plans are developed, including actively controlling the primary disease, supporting the function of each organ, maintaining a stable internal environment, and strengthening disease care management to improve the prognosis of elderly patients with multiple organ dysfunction as much as possible. In order to further improve the quality of medical care and shorten the course of treatment, we cooperate with other professional departments in the hospital, including the clinical nutrition department and clinical pharmacy department, to carry out MDT diagnosis and treatment, and open fast diagnosis and treatment channel services including bedside blood purification (hemoperfusion, plasma exchange, etc.), online hemodialysis treatment, emergency rapid blood transfusion, emergency gastroscopy, DSA examination, CTA, MRI examination, and fully automatic dry chemical analyzer to gain time for early diagnosis and saving lives. It has repeatedly accomplished rescue work for major medical emergencies such as mass trauma and food poisoning, as well as medical support for large-scale conferences and forums in the city. As the former chairing unit of Tianjin Emergency Medicine Quality Control Center and the first aid training base of Tianjin Nursing Quality Control Center, it actively improves the quality of emergency medicine through academic activities, teaching and further training. It takes the lead in holding academic conferences on emergency department quality control and safety and emergency nursing skills training courses every year to improve the academic level of medical care in the industry. Each year, it accepts nearly a hundred trainees, base trainees and on-campus medical students from all over the country, and designates specialized medical and nursing backbones to formulate teaching plans and teaching standards for the trainees, so that they can master corresponding theoretical knowledge and operational skills. Personnel with senior professional titles in the emergency department are responsible for the teaching tasks of Tianjin Medical University and Nankai University School of Medicine.
