Otolaryngology Head and Neck Surgery Department

Tianjin First Central Hospital

The Department of Otolaryngology and Head and Neck Surgery of Tianjin First Central Hospital was founded in 1956 by Professor Lin Bijin, a famous expert in Otolaryngology and Head and Neck Surgery in my country. Over the past decades, it has been trained by Lin Bijin (1911-1996), Yang Baoqi (1931-2017), Zhou Xiangning , Liu Jixiang, Lin Peng, Wang Wei and other generations of department directors, as well as the unremitting efforts and struggles of experts and professors such as Wang Shuhui, Yang Shuolang, Hu Guangai, Zhang Yugeng, Ma Yuanxu, etc., it has always been one of the key disciplines of Tianjin First Central Hospital and a traditional advantage in Tianjin. Combined with Tianjin Institute of Otolaryngology, it has formed a representative specialty in China. The Tianjin Institute of Otolaryngology was established in 1989. In 2018, it was approved as a key clinical discipline in Tianjin. In 2019, it was approved as the Tianjin Otolaryngology Quality Control Center and established the Tianjin Key Laboratory of Hearing, Speech and Balance Medicine. At present, a comprehensive development model of one body and two wings has been established with clinical medicine as the main body and equal emphasis on teaching and scientific research. The comprehensive strength of the discipline is the first in Tianjin, leading in North China, and advanced in the country. There are 91 beds in the discipline, 112 open beds, and a total of 106 medical and nursing technicians. The annual outpatient and emergency visits exceed 120,000, the annual discharge volume is nearly 5,000, and the annual surgical volume reaches more than 6,000.The department is divided into 12 subspecialties: otoneurosurgery, otomicrosurgery, vertigo surgery, otology, rhinology, nasal skull base surgery, rhinal allergy, sleep disordered breathing, voice medicine, thyroid head and neck surgery, maxillofacial head and neck surgery and functional examination department. The functional examination department includes: Dizziness and Balance Center, Hearing Center, Sleep Center, Endoscopy Center, Nasal Function and Allergy Center, and Deafness Gene Testing Center. The institute is divided into: Deafness Molecular Biology Laboratory, Auditory Physiology Laboratory, Speech Pathology Laboratory, Vestibular Physiology Laboratory, Tumor Molecular Biology Laboratory, Epidemiology Research Laboratory, Anatomy Laboratory, Allergy Laboratory, and Nasal Function Laboratory. The discipline team currently has 16 senior students, 4 deputy senior students, 11 doctoral students, 23 master's degree or above, and 7 master's and doctoral supervisors. Since the nose is connected to the base of the skull above, the nasopharynx and palate below, and adjacent to the orbits on the left and right, the diagnosis, treatment and prevention of anatomical, physiological and pathological diseases of the nose and related parts around the nose have become our research direction and content. Although there is still a big gap in some equipment conditions, over the years, under the leadership of Professor Lin Bijin and Professor Yang Baoqi, we have always strived to be consistent with the international and domestic academic development level in terms of overall academic level. The main features of the research direction are: 1. Nasal endoscopic surgery: introduce foreign advanced nasal endoscopic equipment and technology, carry out minimally invasive nasal and sinus surgery, and carry out nose-eye-related and rhinocerebral-related surgeries with the assistance of relevant departments, such as transnasal dacryocystostomy, transnasal orbital decompression, transnasal orbital tumor resection, optic nerve decompression, transnasal cerebrospinal fluid rhinorrhea repair, transnasal sphenoid sinus approach pituitary tumor resection, etc. Under the leadership of Professors Lin Bijin and Professor Yang Baoqi, our department carried out experimental research and clinical application research on pituitary adenoma resection through the septal sphenoid sinus, pioneering transnasal pituitary adenoma surgery in my country. At that time, there was a strong response in the otolaryngology and brain surgery circles, laying the foundation for today's routine transnasal pituitary adenoma surgery. 2. Clinical diagnosis and treatment of paranasal sinus tumors: ⑴ Carry out combined craniofacial approach to resection of malignant tumors in the nasal cavity and paranasal sinuses, and conduct experimental research and clinical application research on resection of advanced craniopharyngioma that invades both nasal cavity and paranasal sinuses at the skull base, providing methods and opportunities for the treatment of advanced nasal cavity and sinus tumors. Comprehensive application of modern high-tech examination and treatment technology to carry out research on the diagnosis and treatment of malignant tumors in the nasal cavity and paranasal sinuses. Research is currently underway to achieve early diagnosis, early treatment, improve curative effect, and improve patients' quality of life.⑵ "Nasal incision and cryotherapy for inverted papilloma of the nasal cavity and paranasal sinuses" solved the problem of postoperative recurrence and won the third prize for scientific and technological achievements at the bureau level. In order to maintain and/or improve the effect, advocate physiological and functional surgery, and improve the quality of life of postoperative patients, a comparative study of conventional nasal incision surgery and endoscopic sinus surgery for inverted papilloma of the nasal cavity and sinuses is currently underway. ⑶Research on micro nasal humidification plugs. In order to reduce the symptoms of nasal dryness and scabbing after sinus tumor surgery, improve the nasal humidification function, and improve the patient's quality of life, research on micro nasal humidification plugs is ongoing. 3. Pterygopalatine fossa and lateral skull base: It is the anatomical space around the nasal cavity and sinuses. Its related diseases and tumors are also research areas where rhinology and otology, head and neck surgery and brain surgery, ophthalmology, radiographic imaging diagnosis and radiotherapy departments collaborate. 4. Achievements: ⑴ "Inferior turbinate fracture displacement surgery" solves the nasal congestion problem of hypertrophic rhinitis, while retaining the structure and function of the inferior turbinate mucosa. It is a functional operation and won the third prize for bureau-level scientific and technological achievements. This technology has been published and promoted and was included in the American Biomedical Abstracts.⑵ "Septoplasty" solves the problem of collapsed nose bridge after septal correction, and also provides a surgical method for the treatment of children's nasal septum deviation. It was rated as a project to fill the technical gap in Tianjin. ⑶ "Endoscopic dacryocystostomy" provides a simple and effective method for the treatment of chronic dacryocystitis and nasolacrimal duct obstruction, filling the technical gap in Tianjin. ⑷ Nasal incision and cryotherapy for reverse papilloma in the nasal cavity and sinuses provide a method to improve the efficacy and reduce recurrence. Won the third prize for bureau-level scientific and technological achievements.
