Hepatobiliary Surgery Department
Tianjin First Central Hospital
The Hepatobiliary Surgery Department of Tianjin First Central Hospital is a clinical department dedicated to the comprehensive treatment of complex hepatobiliary diseases developed under the leadership of Professor Shen Zhongyang, a well-known expert in liver transplantation and liver surgery in my country, on the basis of more than 4,000 cases of liver transplantation and more than 400 cases of related liver transplantation and hepatectomy. The Department of Hepatobiliary Surgery has obvious technical advantages in the precise surgical treatment of liver cancer, the treatment of complex bile and pancreatic diseases, and the treatment of portal hypertension in cirrhosis. It has a team that has been tempered by years of liver transplant surgery. It currently has 4 chief physicians, 6 deputy chief physicians, and 8 people with doctorate degrees. It currently has 1 general ward with 41 beds. The department was the first in China to introduce the IQQA liver CT image interpretation and analysis system from the American company EDDA to accurately measure liver volume. It is equipped with modern liver resection equipment such as 3D laparoscope, CUSA knife, and super suction knife. The latest mixed reality technology (MR) guides liver resection, forming a comprehensive treatment system for hepatobiliary tumors that focuses on precise liver resection and is supplemented by other services such as microwave ablation, interventional therapy, and immunotherapy. The department routinely performs regular liver resection, segmental liver resection, and laparoscopic liver resection without blocking blood flow; For liver cancer patients with severe cirrhosis and portal hypertension, as well as difficult and complicated liver cancer patients with portal vein tumor thrombus or vena cava tumor thrombus, complex and extremely difficult surgeries such as liver cancer resection combined with splenectomy, super-selective portazygotomy, liver cancer resection combined with liver cancer microwave ablation, removal of blood vessels invaded by the tumor, opening of the portal vein stump to aspirate the tumor thrombus, and opening of the hepatic vein to remove the tumor thrombus are performed. Taking advantage of the advantages of the cryopreserved blood vessel bank in the transplant center, we use superb vascular anastomosis technology for tumors with vascular invasion, such as hilar cholangiocarcinoma and pancreatic cancer, and use human cryopreserved blood vessels to replace the blood vessels invaded by the tumor. This gives some patients with tumors considered unresectable by traditional surgery a valuable opportunity for radical surgery.Minimally invasive treatment is one of the development directions of modern medicine. Currently, the minimally invasive treatments routinely carried out by the department include: laparoscopic cholecystectomy, liver resection, pancreatic tumor resection, splenectomy, liver cyst fenestration drainage and other surgeries. It is also the first in China to carry out laparoscopy combined with choledochoscopy and microblast lithotripsy technology. Combining ERCP (endoscopic retrograde cholangiopancreatography) examination and ENBD (endoscopic nasobiliary drainage) to treat hepatolithiasis has the advantages of minimally invasive, repeatable, and few complications. The patient recovers quickly after the operation and the hospitalization time is significantly shortened. It has been unanimously recognized by the majority of patients. Department Features 1. Accurate preoperative assessment, precise and minimally invasive liver resection 1. Preoperative liver volume, tumor volume measurement and surgical path planning: Using IQQA new liver image interpretation and analysis software system and virtual mixed reality technology, the liver volume and blood vessels are accurately assessed before surgery, providing safety guarantee for liver resection and liver transplantation. 2. Measurement of liver reserve function before and after surgery: DDG-300 liver reserve function tester and PICCO liver function reserve module can perform instant detection of liver reserve function during and after surgery. It has important guiding value for the formulation of surgical planning and the prevention of postoperative liver failure.3. Near-infrared fluorescence imaging system navigation for precise liver resection: The Japanese high-efficiency fluorescence imaging device (PDE) is introduced to accurately locate the tumor boundary and detect micro-satellite lesions during the operation. Combined with 3D laparoscopy technology, liver resection surgery can be achieved accurately and minimally invasively. 2. Use living donor liver transplantation technology for radical resection of complex hilar cholangiocarcinoma and gallbladder cancer. The operating room is equipped with an intelligent energy surgery platform, using modern liver resection equipment such as CUSA, super suction knife, and ultrasonic scalpel. It adopts living donor liver transplantation’s fine hilar anatomy and liver splitting technology to perform hemihepatectomy and liver segment resection without blocking the blood flow of the whole liver to perform radical resection of hilar cholangiocarcinoma and gallbladder cancer. This reduces surgical risks and improves the surgical cure rate. 3. To carry out surgeries for advanced liver cancer or pancreatic cancer with vascular invasion or tumor thrombus that cannot be cured by traditional surgery. The transplant center has the unique advantages of cryopreserved human blood vessel banks and superb microsurgical vascular anastomosis technology, using human cryopreserved blood vessels to replace blood vessels invaded by tumors, and to carry out difficult surgeries for liver cancer with vascular invasion or combined with tumor thrombus and pancreatic cancer that cannot be cured by traditional surgery. 4. Combined multi-scope technology for the treatment of complex hepatolithiasis and biliary complications after liver transplantationUsing advanced duodenoscope system, PTCD or PTBD combined with ERCP or ENBD technology, metal covered stents are used to treat hepatolithiasis, biliary strictures and malignant tumors after liver transplantation or in ordinary patients. Laparoscopy combined with duodenoscopy, choledochoscopy, and microblast lithotripsy technology is used to treat extrahepatic and extrahepatic bile duct stones. 5. Routinely perform single-port laparoscopic cholecystectomy, day-case cholecystectomy, and cholelithotomy.
