Cardiovascular Surgery Department

Tianjin First Central Hospital

The Cardiovascular Surgery Department of Tianjin First Central Hospital was established as the main development department of the hospital in February 2009. In order to build the cardiovascular surgery department into a large-scale cardiovascular diagnosis and treatment center and a national heart transplant center in Tianjin. The hospital has invested heavily in equipping cardiovascular surgery with a large number of advanced medical equipment, including American TERUMO extracorporeal circulation machines, anesthesia machines, central monitoring systems that can be connected to ventilator parameters, American DATASCOPE aortic balloon counterpulsation (IABP), 3M extracorporeal membrane oxygenation and centrifugal pumps (ECMO), minimally invasive thoracoscopic systems, intraoperative LUXTEC headlights and camera systems, SCANLAN vascular surgery special surgical instruments and minimally invasive surgical instruments, single-chamber and dual-chamber MEDTRONIC temporary cardiac pacemakers. As well as FRESENIUSKABI autologous blood recovery machine (CELLSAVER) left ventricular assist device and total heart assist device (artificial heart), PET-CT. Large-scale equipment supporting cardiovascular surgery includes ultra-high-speed dual-source CT, 3.0T MRI, ECT, and a full range of immune monitoring systems for organ transplantation. This discipline is led by Director Kong Xiangrong, a well-known cardiac surgery expert and an expert with special allowance from the State Council. The backbone of the department are all Ph.D., master's degree or above, and have many years of working experience in cardiac specialty hospitals, forming a well-trained and high-level professional team, which has laid a solid foundation for the long-term development of this discipline. The cardiovascular surgery department has a total of 24 beds, including 4 independent cardiac monitoring beds. Since its establishment, it has successfully carried out a variety of difficult surgeries, such as: heart transplantation, coronary artery bypass grafting for 80-year-olds, left atrial volume reduction and pulmonary vein isolation to treat valvular disease combined with intractable atrial fibrillation, combined replacement of the aortic valve, mitral valve, and tricuspid valve plus coronary artery bypass grafting; double valve replacement for severe rheumatic heart disease combined with chronic renal insufficiency and severe pulmonary hypertension. There are also emergency aortic valve replacement plus vegetation removal and coronary artery bypass grafting for difficult-to-control subacute bacterial endocarditis combined with coronary artery disease, emergency coronary artery bypass grafting plus ventricular aneurysm resection, ascending aorta replacement plus coronary artery transplantation for severe aortic stenosis combined with ascending aortic aneurysm, radical surgery for severe tetralogy of Fallot in infants and young children, and secondary radical surgery after palliation for extremely severe tetralogy of Fallot.Coronary artery bypass surgery plus diaphragmatic hernia repair for severe coronary heart disease combined with huge septal hernia after multiple cerebral infarctions and myocardial infarctions, off-pump coronary artery bypass surgery for severe coronary heart disease combined with ischemic cardiomyopathy and severely reduced left ventricular function (EF value 28%), minimally invasive congenital heart disease surgery with a small incision under the right armpit and a midline half-incision on the sternum. Ascending aortic replacement (Bentall) plus total aortic arch replacement, total aortic arch replacement plus distal stent grafting, stent grafting for dissecting aneurysms through the common iliac artery, and stent grafting for dissecting abdominal aortic aneurysms. The surgeries performed cover almost all areas of cardiovascular surgery. Many of these difficult surgeries cannot be performed by other hospitals in our city, including specialized hospitals. Many patients came to the cardiovascular surgery department for treatment and were successfully discharged after being told they could not operate in other large hospitals. Working hard, forging ahead and striving for excellence have made the cardiovascular surgical treatment level of this center among the advanced ranks. Features of the department 1. Heart transplant surgeryDepartment Director Kong Xiangrong began to engage in clinical work in heart transplantation and heart-lung transplantation in 1991. In 1992, he participated in the completion of 2 heart transplants and 2 heart-lung transplants in China. Heart transplantation won the third prize of the Ministry of Health's Life Science and Technology Progress Award. In 2000, he went to Australia's Prince Charles Hospital for special training in heart transplantation and coronary artery bypass grafting. In the past 20 years, he has participated in and performed more than 50 heart transplants. Currently, as an advanced heart transplant center in Tianjin, this department will provide first-class services to more patients with advanced heart failure and give more patients with severe heart disease a new lease of life. Relying on the equipment and technical support of our organ transplant center, it is a center that can perform heart transplants, heart-lung transplants, and heart-kidney transplants at the same time. 2. Director of Coronary Heart Surgery Department Kong Xiangrong began to independently perform coronary artery bypass surgery under extracorporeal circulation in 1994, and started beating heart coronary artery bypass surgery in 1997. He is a well-known expert in the field of coronary heart surgery in China. At present, in addition to conventional coronary artery bypass grafting, our department has successfully completed coronary artery bypass grafting for those over 80 years old and made it a routine operation in the department, and completed off-beat bypass surgery for severe coronary heart disease combined with ischemic cardiomyopathy and severely reduced left ventricular function;A series of high-risk and difficult surgeries include bypass surgery for patients with severe coronary heart disease and huge ventricular aneurysm and resection of ventricular aneurysm; non-stop bypass surgery for patients with advanced coronary heart disease and uremia; bypass surgery for severe coronary heart disease and huge diaphragmatic hernia after multiple cerebral infarctions and myocardial infarction plus diaphragmatic hernia repair. 3. Valvular Surgery In 2005, left atrial volume reduction and pulmonary vein isolation were carried out to treat valvular disease combined with refractory atrial fibrillation. The successful implementation of this operation not only enabled a large number of patients with valvular disease combined with atrial fibrillation to be cured, but more importantly, enabled the patients to save medical expenses. After this technology was published in the Chinese Journal of Thoracic and Cardiovascular Surgery in 2007, it aroused strong repercussions in society. At the same time, new technologies such as radiofrequency ablation for atrial fibrillation and microwave treatment for atrial fibrillation were launched earlier in the country. This discipline rationally selects valvuloplasty or valve replacement based on the causes and pathological changes of patients' valvular diseases. So far, the success rate of valvuloplasty is 100%. This department performs valve replacement surgery for various valve diseases, including combined replacement of the aortic valve, mitral valve, and tricuspid valve plus coronary artery bypass grafting for multiple valve diseases combined with coronary artery disease;Difficult and critical surgeries such as double valve replacement for severe rheumatic heart disease combined with chronic renal insufficiency and severe pulmonary hypertension. 4. The Department of Congenital Heart Disease Surgery successfully performed aortic transposition surgery for newborns with complete transposition of the great arteries in Tianjin in 2004. At that time, only a few cardiovascular centers in China could perform this operation in small quantities. Independently researched and invented the vena cava-aortic bypass method to perform the Glenn operation and the total vena cava-pulmonary artery connection. The results were published in the Chinese Journal of Thoracic and Cardiovascular Surgery in 2009, filling the gaps in this technology at home and abroad. Various corrective surgeries for complex congenital heart diseases have been carried out, among which the success rate of surgeries for tetralogy of Fallot is as high as 99.5%. 5. The Department of Vascular Surgery began to perform ascending aorta replacement (Bentall) plus total aortic arch replacement, total aortic arch replacement plus distal covered stent placement in 2000. David, Bentall and other surgeries are at an advanced level. Among them, interventional stent placement or aortic vessel replacement for emergency aortic dissection has saved the lives of countless patients. 6. Minimally invasive surgerySince 1997, he has successfully completed atrial septal defect and ventricular septal defect repair through small incisions in the lower sternum, mitral valve replacement through small incisions in the right axilla and right anterolateral side, repair of atrial septal defect and ventricular septal defect, and aortic valve replacement through small incisions in the upper sternum. It is worth mentioning that many minimally invasive surgeries have become routine surgeries in this discipline based on the patient's condition and aesthetic needs. 7. Interventional closure. In 2006, minimally invasive patent ductus arteriosus closure via ultrasound-guided small chest incision was carried out, which completely avoided the radiation exposure of traditional closure surgery and brought good news to the majority of patients with congenital heart disease and their families. Now this surgery has become a routine surgery and has been promoted to major hospitals in China. At the same time, many cases of atrial septal defect and ventricular septal defect closure were successfully performed. We performed stent grafting for dissecting aneurysms via the common iliac artery, and successfully completed stent grafting for abdominal dissecting aneurysms with a small aneurysm neck (1.1cm) near the renal artery opening.
