Department of Infectious Diseases

Tianjin First Central Hospital

The Department of Infectious Diseases of Tianjin First Central Hospital is a clinical department established by our hospital in 2004 in response to the call of the Ministry of Health, integrating medicine, teaching and research. It is an independent clinical department specializing in the diagnosis and treatment of infectious diseases. It is a professional department that researches, diagnoses and treats various infectious diseases (non-infectious diseases) and difficult and complicated diseases. Although the Department of Infectious Diseases is an interdisciplinary subject and has been established for a short period of time, coupled with the deviation of patient understanding and the young team of doctors, relying on the advanced equipment and technical support of our hospital, the medical staff of the department work together, starting from standardizing various medical behaviors, focusing on quality and service, the overall level of the department has achieved rapid development, and the status and influence of the department are constantly increasing rapidly. At the beginning of the establishment of the Department of Infectious Diseases, there were only 15 beds and poor ward conditions. Under the leadership of the old director Wei Jingping, we overcame various difficulties and accumulated rich experience in the diagnosis and treatment of refractory pneumonia, infections with low immunity such as diabetes, advanced age, infections after organ transplantation, skin and soft tissue, orthopedic surgery, and urinary tract infections. With the support of the dean and the party committee of the hospital, the number of fixed beds in the infectious disease department has been increased to 27, and the ward environment has been greatly improved.Under the leadership of current department director Yang Wenjie, the Department of Infectious Diseases has paid more attention to discipline construction, strictly implemented various core systems and standardized management, and implemented the concept of "patient-centered, technology-based support, and service-based". Economic income and medical indicators have been greatly improved. It can be said that the Department of Infectious Diseases has entered the fast lane of discipline development. The diseases treated by the Department of Infectious Diseases are mainly infectious diseases and febrile diseases of unknown origin. It involves every organ and system in the body and overlaps with many clinical departments of our hospital. However, severe infections, infections in immunocompromised people, infections with drug-resistant bacteria, fungal infections and refractory infections have formed the characteristics of the department. There are many types of unexplained febrile diseases, making diagnosis difficult, and many patients have been transferred to multiple hospitals. The Department of Infectious Diseases attaches great importance to the introduction and development of new technologies, makes full use of the existing medical resources of our hospital, and closely cooperates with the blood culture room, fungus room, isolation and identification room, drug susceptibility room, molecular room, and immune room. It has imported advanced equipment and instruments. Among them, bacterial and fungal culture and identification, identification and drug sensitivity of Candida, Aspergillus, Mucor, and tuberculosis-activated T lymphocyte interferon release experiments have reached the level of domestic advanced laboratories. The Department of Infectious Diseases works closely with the Department of Radiology, and has advanced 16-slice multi-slice spiral CT, 1.5T magnetic resonance and whole-body PET/CT. The scanning time is short and fast, and it only takes 7 seconds to image a moving part. CT endoscopy can non-invasively detect polyps, tumors and other lesions inside and outside the cavity, and can clarify the scope of the disease. Based on the different doubling times of small nodules in the lungs, the benign and malignant lesions can be determined. It works closely with the respiratory department and gastroenterology department in the hospital, and has advanced electronic bronchoscopy, pulmonary function meter, allergen examination, gastrointestinal endoscopy and other equipment. Advanced equipment provides high-quality and efficient help for the diagnosis of unexplained fever, greatly improving the disease diagnosis rate. The diagnosis rate has reached more than 95%, and has formed a certain social influence. While the Department of Infectious Diseases is engaged in the daily work of teaching and treating patients, it is also responsible for the management of intestinal clinics, fever clinics and fever wards during influenza outbreaks, a large number of consultations for infectious diseases in various clinical medical departments, and out-of-hospital consultations. At the same time, we work closely with the Pharmacy Department, Preventive Health Care Department, and Microbiology Laboratory to assist the Medical Department, actively carry out epidemiological surveys and risk factor analysis of hospital infections, monitor bacterial resistance, strengthen hospital infection control, and supervise the rational application of antibacterial drugs to reduce the occurrence of drug-resistant bacterial infections and the outbreak of hospital infections. The Department of Infectious Diseases currently has 21 medical staff, including 10 physicians, 1 chief physician, 3 deputy chief physicians, and 4 attending physicians. 70% of the doctors have a master's degree or above, 45% of the nurses have a bachelor's degree, and 36% are undergraduate students. The current subject leader is Director Yang Wenjie, a master's tutor, and the deputy director of the Prevention and Construction Department of our hospital. He is currently a member and secretary of the Infectious Diseases Group of the Tianjin Respiratory Society, a member of the Standing Committee of the Tianjin Allergy Society, a member of the Tianjin Infection Quality Control Center, a youth member of the Chinese Society of Integrated Traditional Chinese and Western Medicine Allergy, a member of the Tianjin Public Health Emergency Emergency Expert Group, an expert of the Tianjin Influenza A (H1N1) Municipal Expert Group, and an expert of the Tianjin Municipal Health Bureau’s Rational Application of Antimicrobial Drugs Expert Steering Group. Director Zhao Xuequn is a member of the Tianjin Respiratory Society of Integrated Traditional Chinese and Western Medicine, and Director Liu Jun is a youth member of the Tianjin Infection Society.Director Yang Wenjie was rated as an outstanding party worker in the Tianjin Health Bureau system in 2011, and head nurse Xiao Li was awarded the title of "Good Nurse" in the Tianjin Health System in 2010. In terms of scientific research, we pay attention to the improvement of academic standards: in the past two years, we have published 3 articles in Chinese series of magazines, 10 articles in core journals, participated in the compilation of 3 monographs published by People's Medical Press; participated in 4 international academic conferences, 20 domestic academic conferences, and 2 special lectures sponsored by the Ministry of Health. In order to improve the academic qualifications of young doctors, young doctors are sent to participate in high-level academic conferences every year to guide the topic selection and writing of papers. Teaching situation: It is responsible for some teaching tasks of Nankai University School of Medicine and Tianjin Medical University's five-year undergraduate internal medicine, as well as all teaching tasks of internal medicine of Tianjin Medical University Night College. It is a standardized training base for resident doctors in our city, training more than 60 people every year. At the same time, he is responsible for the clinical teaching work of trainees and interns at Tianjin University of Traditional Chinese Medicine, and teaches more than 100 interns every year. In 2011, it was approved by the Ministry of Health as a clinical pharmacist training base. It is currently the master’s training center of Tianjin University of Traditional Chinese Medicine. A municipal continuing education lecture is held every year to systematically explain the rational application of antibacterial drugs. After the launch of the Ministry of Health's special management campaign on the rational use of antimicrobial drugs, training on antimicrobial drug management was conducted in many hospitals in our city. Due to the continuous discovery and understanding of infectious diseases caused by new pathogenic microorganisms, the complexity of bacterial resistance mechanisms, and the high incidence and mortality of infectious diseases, the clinical diagnosis and treatment of infectious diseases still faces severe challenges. Under the care and guidance of hospital leaders and driven by the hospital's great development, the Department of Infectious Diseases will strengthen discipline construction, standardize management, strictly implement various core systems, and rely on technology to improve the diagnostic methods for the etiology of infectious diseases. For example, an infectious disease diagnostic laboratory should be established to carry out standardized quantitative diagnosis and treatment under bronchoscopy, improve the genetics, molecular biology and serological testing of viruses, atypical pathogens and fungi, and carry out detection of cytokines and inflammatory mediators to further improve the diagnostic level of patients with unexplained fever and reduce the irrational use of antibacterial drugs. Raise scientific research awareness, carry out scientific research in various aspects, strengthen talent training, establish an education and training system for department personnel, carry out multi-level cooperation and exchanges with domestic and foreign domestic and foreign colleagues in the same field and related disciplines, pay attention to the improvement of technical level, and serve patients and society with high-quality services and superb skills. The features of the department are: 1. Fever Clinic: Responsible for the initial diagnosis of all febrile diseases and the screening of infectious febrile diseases, and performs diagnosis, differential diagnosis and treatment of various common types of febrile diseases. The specific opening time will be notified that year, and the nature of the 24-hour emergency service will be announced. 2. Intestinal Clinic: The treatment scope covers all intestinal diseases, including diagnosis and treatment of intestinal diseases with symptoms such as diarrhea and abdominal pain. It is also responsible for the monitoring, treatment, prevention and control of intestinal infectious diseases. The clinic is open from May 1st to October 31st every year, and the emergency work is 24 hours a day. 3. Infectious disease clinic (non-infectious disease): It mainly diagnoses and treats patients with various infectious diseases (pneumonia caused by bacterial, fungal, and viral infections, digestive system infections, urinary tract infections, etc.), as well as patients with unknown causes of fever. Clinics are open on weekdays throughout the year, and the work nature of expert outpatient clinics is on weekdays.
