Hematology Department
Tianjin First Central Hospital
The Hematology Department of Tianjin First Central Hospital is a clinical department integrating medicine, teaching and research. The department currently consists of three parts: the general ward, the bone marrow transplantation ward and the hematology research laboratory. It mainly treats hematopoietic system and lymphoid tissue diseases and other systemic diseases complicated by serious blood system diseases, including: hemolytic anemia, aplastic anemia, iron deficiency anemia, megaloblastic anemia, aplastic anemia, acute leukemia, and chronic leukemia. , Myelodysplastic syndrome, myeloproliferative neoplasms, lymphoma, multiple myeloma, immune thrombocytopenia, thrombotic thrombocytopenic purpura, leukopenia, coagulation abnormalities, hypersplenism, bone marrow metastasis cancer, acute bone marrow necrosis, etc. The Hematology Department is a well-known hematology diagnosis and treatment center in the country. It has formed a hematology diagnosis and treatment center featuring precise and individualized systematic diagnosis and treatment of elderly hematological tumors (leukemia, lymphoma, myeloma), tumor immunotherapy based on CART cells, and haploid hematopoietic stem cell transplantation. It is a designated unit for hematopoietic stem cell collection and transplantation by the Chinese Bone Marrow Bank and the Red Cross Society of China. It is a unit qualified for bone marrow transplantation. It is a dominant discipline in Tianjin. It was inspected as a key discipline in the re-evaluation of Tianjin's tertiary-level hospitals and passed expert recognition. The department has a total of 65 staff, including 1 doctoral supervisor and 2 master's supervisors. There are 25 doctors, 36 nurses, and 4 technicians; professional structure: 13 people with senior professional titles, 27 people with intermediate professional titles, and 25 people with junior professional titles. There are currently 8 doctors and 19 masters. The department has 3 hospital wards: Fukang Road Campus has 62 designated beds, including 6 laminar flow wards; Shuixi Campus, which will be put into use at the end of 2021, has 104 designated beds, including 10 laminar flow wards; and the East Campus (Tianjin Workers' Hospital/Tianjin Occupational Disease Prevention and Control Hospital) has 21 designated beds. Chief Physician Li Yuming, the former director of the Department of Hematology, is now the subject leader. Chief physician Li Yuming is the vice chairman of the Tianjin Hematology Branch of the Chinese Medical Association, a standing member of the Chinese Medical Biotechnology Clinical Application Professional Committee, and a specially appointed guiding expert of the Chinese Bone Marrow Bank. Chief physician Zhao Mingfeng is currently the deputy director of the Department of Hematology and presides over the work of the department. Chief physician Zhao Mingfeng is a doctoral tutor at Nankai University and Tianjin Medical University, a first-level candidate for Tianjin’s “131” innovative talents, and the first Tianjin “Tinmen Medical Talent”.He has profound knowledge in targeted therapy and individualized treatment of hematological tumors, haploid hematopoietic stem cell transplantation, cellular immunotherapy (especially CART cellular immunotherapy) and hematological tumors, and is a well-known expert in this field in China. Chief physician Deng Qi is currently the deputy director of the Department of Hematology and secretary of the party branch. Chief physician Deng Qi is a postgraduate tutor at Tianjin Medical University. He is good at the diagnosis and treatment of various hematological diseases, hematopoietic stem cell transplantation technology and biological immunotherapy of malignant hematological tumors. Features of the department include: 1. Systematic diagnosis and treatment of hematological tumors (leukemia, lymphoma, myeloma) in the elderly: The three common hematological tumors (leukemia, lymphoma, myeloma) are highly prevalent in the elderly, so the treatment of hematological tumors in the elderly plays an important role in the diagnosis and treatment of blood diseases. Relying on the comprehensive advantages of the First Center, the Hematology Department of the First Center is at the forefront of Tianjin in the diagnosis and treatment of hematological tumors in the elderly. With the funding of the Tianjin Health Industry Key Research Project, by leading a number of blood centers in Tianjin, we use individualized stratification, multi-index detection of minimal residual disease (MRD) in tumors, and new drug regimens combined with cellular immunotherapy, and develop individualized comprehensive treatment plans suitable for elderly hematological tumors based on MRD levels. Most elderly patients with hematological tumors can receive active treatment, which greatly improves the efficacy of hematological tumors in the elderly. 2. Immunotherapy of hematological tumors (CIK cells, DC-CIK cells, NK cells, CART cells): With funding from the National Natural Science Foundation and other projects, the Hematology Department carried out CIK and DC-CIK clinical trials for the treatment of minimal residual disease in leukemia earlier in Tianjin and achieved good results, and won the 2013 Tianjin Science and Technology Progress Award.CART cell therapy is a milestone in today's biological immunotherapy. It has very good curative effect on some relapsed and refractory hematological tumors after radiotherapy and chemotherapy. Since 2015, the hematology department of a center has carried out CART cell therapy for refractory and relapsed hematological tumors (leukemia, lymphoma, myeloma). It is one of the earliest centers in China to carry out and complete the largest number of patients (it has completed the treatment of nearly 600 patients so far), and the curative effect has reached the international advanced level. He has published a number of SCI articles with high impact factors, and has received funding from key research projects in the Tianjin health industry and the National Natural Science Foundation of China. The Department of Hematology has made great achievements in CART cell therapy, and has pioneering and successful experience. For example, the use of plasma exchange to treat severe cytokine storm reactions has been written into the consensus of treatment experts; it was the first to report the use of CART cells to solve central nervous system tumor invasion. For refractory central nervous system leukemia that has been ineffective for repeated intrathecal infusions or difficult to control with conventional treatments, the use of intrathecal chemotherapy combined with CART cell therapy has solved this central nervous system invasion with good safety. At the same time, there is also exploratory work in the field of cell therapy, and it is found that CART cells combined with immune checkpoint inhibitors can achieve better efficacy and improve the complete remission rate in patients with refractory relapsed lymphoma. Before hematopoietic stem cell transplantation, the infusion of CART cells derived from fully synchronized donors was more effective than those derived from semizygous donors. It also proposed a new concept of CART treatment. For patients with minimal residual disease (acute lymphoblastic leukemia or lymphoma), it is recommended to advance the treatment time of CAR-T cells, which has good efficacy and few side effects. 3. Haploidentical haploidentical hematopoietic stem cell transplantation: In 2008, the Department of Hematology carried out full-identity sibling allogeneic hematopoietic stem cell transplantation for the treatment of acute leukemia and achieved complete success. The patient has long-term disease-free survival to this day. Later, we successively carried out allogeneic hematopoietic stem cell transplantation or autologous hematopoietic stem cell transplantation for patients with acute myeloid leukemia, acute lymphoblastic leukemia, chronic myelogenous leukemia, myelodysplastic syndrome, malignant lymphoma, acute aplastic anemia, etc. We have accumulated rich clinical experience, achieved good therapeutic effects, and saved the lives of patients. One of the main problems faced by hematopoietic stem cell transplantation is the insufficient source of donors, and many patients lose the opportunity for transplantation.Haploid haploidentical transplantation can solve this problem very well. But there are high demands on the implementation team. The Hematology Department of a center has been carrying out haploidentical haploidentical hematopoietic stem cell transplantation since 2015, and has successfully completed transplants for hundreds of patients. By improving the pretreatment plan and the application of third-party cord blood, the number of implants has increased, rejection has been reduced, and similar efficacy to that of full-identity transplantation has been achieved. In order to reduce recurrence and improve the efficacy of transplantation, the Hematology Department of a center has done a lot of pioneering work. For example, for patients with high-risk acute lymphoblastic leukemia, CD19-CART cells were prepared from donor-derived cells after transplantation and carried out preventive infusion, which effectively solved the problem of recurrence after transplantation. For patients with high-risk acute myeloid leukemia and myelodysplastic syndrome, the Department of Hematology is the first at home and abroad to carry out post-transplantation targeted drug maintenance treatment, using demethylating drugs plus BCL-2 inhibitor treatment, which is safe and effective, greatly reducing recurrence and improving the success rate of transplantation.
