Breast Surgery Department

The First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine

The Breast Surgery Department of the First Affiliated Hospital of Tianjin University of Chinese Medicine is an integrated traditional Chinese and Western medicine surgery featuring breast specialty. Based on our hospital's more than 50 years of experience in treating breast diseases with traditional Chinese medicine, it highlights the characteristics of "minimally invasive, traditional Chinese medicine". Currently, there are 9 staff in the department, including 4 chief physicians, 1 deputy chief physician, 4 attending physicians; 2 doctors, 3 masters, and 4 undergraduates. 1. Current diseases: Breast inflammatory diseases (acute lactation mastitis, granulomatous mastitis, plasma cell mastitis); Breast proliferative diseases (breast hyperplasia, mastalgia, breast adenosis); Breast congenital malformations (nipple inversion, accessory breast, abnormal male breast development, etc.); Breast tumor diseases (breast fibroadenoma, intraductal papilloma, breast cancer). 2. Specialties, special diseases, special treatments, and special drug treatments 1. Minimally invasive treatment and biopsy of breast tumors: Widely used in breast fibromas, breast hyperplasia nodules, breast cysts, breast fibrocalcifications, lipomas, gynecomastia, accessory breasts, multifocal breast lesions, unknown masses, etc. The application of this system further improves the early diagnosis rate of breast diseases, breaks the routine of breast disease treatment, and not only solves breast diseases, but also preserves women's perfect breasts.2. Lactation mastitis: It is a disease treated with the characteristics of traditional Chinese medicine in the department. Traditional Chinese medicine treatments such as oral administration of traditional Chinese medicine, external treatment, and fire acupuncture to drain pus are adopted. ⑴Oral administration of traditional Chinese medicine: Individualized chemotherapy will be given according to different syndrome differentiation stages, such as mass stage, pus stage, drainage or ulceration, and prescription treatment for breast reflux will be given according to the condition and patient requirements. ⑵External treatment methods: According to different syndrome differentiation and staging, TCM methods such as bandaging, puncture and pus drainage, fire needle drainage, incision and drainage, putrefaction and muscle regeneration, cotton padding and other methods are applied respectively, together with corresponding specialist external drugs and disease-specific dressings. ⑶Traditional fire needle drainage: hole-type incision, small damage, does not affect breastfeeding, and reduces complications such as breast fistula. ⑷Breast massage: Nurses with professional prolactin qualifications set up dedicated breast massage clinics to better protect patients' privacy. ⑸Treatment and prevention clinics: Open pregnancy breast health care and lactation health consultation clinics, and psychological counseling clinics.3. Non-lactation mastitis (granulomatous mastitis, plasma cell mastitis): At present, a variety of treatment methods for this disease have been carried out, and nearly hundreds of cases have been accumulated. A relatively standardized series of treatments has been formed, such as preoperative and postoperative traditional Chinese medicine treatment, external treatment, selection of external medicines, methods of lesion removal (reversible gland-preserving debridement, VSD negative pressure drainage), one-stage and two-stage suture-free sutures to maintain the shape of the breast, etc., which have formed the department's specialty therapy. 4. Breast hyperplasia and mastalgia: A routine series of treatments has been formed, including a series of agreed prescription drugs and acupuncture, cupping, application therapy, thread embedding, physical therapy and other therapies. Specialized medicines: Rutongling No. 1, No. 2, Huahe Pills, Ruzenkang Decoction, Coptidis Coptidis Paste, Jinhuang Paste, etc. 5. Diseases within the breast ducts: Breast duct angiography, segmental breast resection and other surgeries have been carried out. In view of the fact that intraductal lesions are currently limited to exfoliated cell examination, traditional Chinese medicine treatment and breast segment resection, the surgical trauma is relatively large, and the positioning of the diseased ducts is not accurate, which has the disadvantage of expanding the scope of resection. In addition, intraductal papilloma itself has a high possibility of malignant transformation, and preoperative diagnosis is limited. Breast fiber ductoscopy is planned to be introduced to better inspect, biopsy, diagnose and minimally invasive treatment of intraductal lesions.6. Congenital malformations of the breast: Surgery for breast malformations mainly includes correction of inverted nipples, abnormal male breast development, and accessory mastectomy. Graded treatment for nipple inversion: Grade I patients with inverted nipples choose conservative treatment such as nipple orthotics; Grade II patients, especially those who require re-lactation, choose nipple braces or orthopedic surgery; Grade III patients undergo a special surgery - modified nipple incision plus "Z"-shaped flap transfer and correction. The short-term and long-term results are good, especially for patients who do not require re-lactation. 7. Malignant breast tumors: Radical surgery, neoadjuvant chemotherapy, traditional Chinese medicine treatment, and endocrine therapy for breast cancer have been carried out for many years. While continuing to improve radical surgery, we are planning to make breast cancer surgery minimally invasive, carry out sentinel lymph node biopsy for breast cancer, reduce the number of axillary lymph node dissection cases, and make the treatment more humane. While ensuring the curative effect, we will use our hospital's endoscopic system to carry out mastoscopic axillary lymph node dissection to reduce patient pain and postoperative complications.8. Surgery and minimally invasive treatment of general surgical diseases: In order to improve the comprehensive surgical strength of the two hospitals, minimally invasive surgery for inguinal hernia and preperitoneal space repair, difficult hernia surgery, minimally invasive treatment of hemorrhoids, minimally invasive surgery for rectocele, diagnosis and surgical treatment of common acute abdomen, and early postoperative intervention with traditional Chinese medicine have achieved good results.
