Hepatobiliary Department

The First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine

The Department of Hepatobiliary Diseases of the First Affiliated Hospital of Tianjin University of Chinese Medicine is a key specialist unit for liver diseases of traditional Chinese medicine in Tianjin. It is responsible for the clinical diagnosis, treatment, teaching and scientific research of the prevention and treatment of hepatobiliary system diseases with traditional Chinese medicine in Tianjin. In 2012, it was recognized as the "National Fatty Liver Diagnosis and Treatment Center" by the State Administration of Traditional Chinese Medicine. It is currently the chairman unit of Tianjin Professional Committee for Integrated Traditional Chinese and Western Medicine in Liver Disease and the deputy chairman unit of Tianjin Professional Committee of Digestion of Traditional Chinese Medicine. The discipline carries out special diagnosis and treatment around four major diseases: comprehensive prevention and treatment of fatty liver disease and metabolic syndrome, integrated diagnosis and treatment of liver cirrhosis with traditional Chinese and Western medicine, integrated diagnosis and treatment of liver cancer with traditional Chinese and Western medicine, and research on TCM treatment plans for autoimmune liver diseases. Acute and chronic hepatitis of various causes (hepatitis B, hepatitis C, alcoholic liver disease, drug-induced liver damage, autoimmune liver disease), cirrhosis, primary and secondary liver cancer, pancreatic and bile duct tumors, liver cysts, liver abscesses, liver hemangioma, gallbladder stones, acute and chronic cholecystitis, etc. 1. Comprehensive treatment with traditional Chinese medicine: Traditional Chinese medicine treatment (drug wet compress, acupoint application), external TCM treatment (Zhushui Patch, Spring Liver Nourishing Patch), etc. are carried out to treat refractory ascites and severe abdominal infection, with good curative effect and outstanding characteristics.Cupping and scraping in traditional Chinese medicine can promote qi and blood circulation, warm yang and dispel cold, and improve patients' digestive system symptoms such as hypochondriac pain, abdominal distension, diarrhea, and indigestion. In addition, acupuncture, acupoint application, acupoint cupping, and auricular acupuncture are carried out in outpatient clinics for patients with fatty liver and metabolic syndrome to achieve the purpose of strengthening the spleen, removing dampness and turbidity, and helping patients lose weight. 2. Selective hepatic arterial chemoembolization Selective hepatic arterial chemoembolization (TACE) is a minimally invasive surgery and is currently the preferred treatment method for mid-to-late stage liver cancer that cannot be treated by surgery. TACE mainly blocks the blood supply of the tumor by embolizing the blood supply artery of the tumor, causing tumor ischemia and hypoxia, thereby inhibiting tumor growth and promoting tumor cell necrosis and apoptosis. The whole process of TACE treatment is carried out under the guidance and supervision of imaging equipment. It can accurately and directly reach the lesion through tiny wounds, so it has the advantages of minimally invasive, precise, safe and efficient. 3. Percutaneous transhepatic biliary drainage Percutaneous transhepatic biliary drainage refers to a series of techniques that use imaging equipment (usually X-ray fluoroscopy or B-ultrasound under DSA) to percutaneously puncture the bile duct and insert a drainage tube to allow bile to flow outside the body or into the duodenum. Mainly used for the treatment of biliary obstruction and acute inflammation.4. Biliary duct stent implantation As the name suggests, it is to place a stent in the biliary tract to open the obstructed biliary tract and restore normal bile flow. Cholangiocarcinoma, pancreatic head cancer, periampullary tumors, and parabiliary metastases are common diseases of the digestive system. Compared with endoscopic retrograde biliary stent implantation, percutaneous biliary stent implantation is simple to operate. It is a puncture technique and can be performed under local anesthesia. You can get out of bed and move around on the day of the operation. It unblocks the biliary tract, allowing bile to flow into the intestine through normal physiological channels, reducing jaundice and restoring liver function. The incidence of complications of this operation is significantly lower than that of surgery. The expandable memory metal stent makes the puncture path more minimally invasive. 5. Indications for radiofrequency ablation of liver cancer 1. Single tumor with maximum diameter ≤5cm; or number of tumors ≤3 with maximum diameter ≤3cm. 2. There is no vascular tumor thrombus or invasion of adjacent organs. 3. Liver function grade Child-pughA or B, or reach this standard after medical treatment. 4. For single tumors with a diameter >5cm that cannot be surgically removed or multiple tumors with a maximum diameter >3cm, radiofrequency ablation can be used as part of palliative treatment or combined treatment. Advantages: First: minimally invasive. Second: The curative effect is very accurate, and it can achieve a radical cure effect for tumors smaller than 3 cm.Third: Recovery is fast, and patients can get out of bed and move around quickly after surgery. Fourth: The requirements for patients are lower. 6. Percutaneous liver puncture and drainage of liver cysts and liver abscesses Liver cysts are benign lesions in the liver. However, if the cyst is too large and the cyst ruptures, compression symptoms will occur and liver function will be affected. Percutaneous liver puncture and drainage of liver cysts can relieve compression symptoms and has the advantages of fast recovery, less trauma, and less bleeding. Liver abscess is a serious infectious disease that can cause dysfunction of the liver and multiple organs throughout the body. It generally requires timely and adequate anti-infective treatment. Liver abscess puncture and drainage can shorten the course of medication, improve treatment effects, and reduce hospitalization time. Special disease introduction 1. Autoimmune liver disease Autoimmune liver disease refers to a group of liver diseases in which the direct cause of liver damage is autoimmune reaction rather than viral infection. It mainly includes autoimmune hepatitis, primary biliary cholangitis and primary sclerosing cholangitis. We look for new directions based on old foundations. Old foundation: Carry out clinical research on the treatment of autoimmune liver disease with traditional Chinese medicine "Nourishing the kidney, softening the liver and activating blood circulation", and seek the best treatment plan for autoimmune liver disease with traditional Chinese medicine. Actively carry out the attenuation and synergy effects of hormone therapy on people with autoimmune hepatitis.New direction: Aiming at the diagnosis and treatment of drug-induced liver damage, especially drug-induced autoimmune liver damage, explore and develop integrated traditional Chinese and Western medicine diagnosis and treatment options. It also cooperates with out-of-hospital research units to carry out mechanism research. 2. Fatty Liver Disease The Department of Hepatobiliary Disease is one of the national standardized diagnosis and treatment centers for fatty liver. It has one permanent specialized clinic and receives more than 5,000 patients with fatty liver in half a year. It has two permanent chief physicians who work on a 6-day-a-week consultation system and treats nearly 1,000 fatty liver patients every month. The outpatient clinic has one open examination room, which is mainly used to collect patient health data and liver elasticity testing; one treatment room is ready to be opened to provide patients with external TCM treatment. The total value of the equipment purchased by the specialist in the past five years has reached more than 2 million yuan. It is equipped with a set of FibroScan fatty liver detection system testing equipment from the French company Echosens, a body composition analyzer, an abdominal color ultrasound, a fully equipped work computer, printing equipment, a body fat scale, and file cabinets, providing a one-stop diagnosis and treatment model for patients with fatty liver. And use new media and other forms to carry out health education and disease population management.
