Breast Oncology Department 3

Tianjin Medical University Cancer Hospital

The Third Department of Breast Oncology of Tianjin Medical University Cancer Hospital was officially established in 1972. It was the first to carry out extended radical mastectomy for breast cancer nationwide. With the development of early diagnosis of breast cancer, it also performed modified radical mastectomy for breast cancer with preservation of pectoral muscles, laying the foundation for the nationwide implementation of this surgery.

With an in-depth understanding of the biological characteristics of breast cancer, we took the lead in carrying out research on comprehensive treatment of breast cancer in China and achieved significant results in clinical breast cancer treatment, thus establishing our hospital's leading position in breast cancer research in the country.

At the same time, nurses in the department began to conduct research on functional recovery exercises for the affected upper limb after breast cancer surgery.

After decades of development, it further expanded and established the Asian Breast Cancer Prevention and Treatment Center at the end of 2003. The first, second and third breast departments have a total of 133 beds, with an annual outpatient volume of 25,000 and an annual treatment of 2,000 cases of breast cancer.

While strengthening its own development, the Department of Breast Surgery keeps pace with international progress and integrates early diagnosis of primary breast cancer, clinical treatment, pathological diagnosis and basic research, becoming a leading and leading unit with absolute advantages in the field of breast cancer research nationwide.

1. The Breast Center has strong capabilities and rich experience in diagnosis and treatment.

The Third Department of Breast Oncology was formally established in 2003, with Professor Zhang Jin as the director. Since its establishment, it has standardized the treatment of breast cancer with reference to international standards.

The five-year survival rates of clinical stage I and II breast cancer are 94% and 83% respectively, and the ten-year survival rates are 87% and 67% respectively, both of which are among the leading domestic levels and in sync with the international level in the same period.

There are 46 beds in total, and the expert team consists of 2 chief physicians and 3 deputy chief physicians. They have rich clinical experience and are particularly unique in the diagnosis and treatment of breast cancer. They not only treat the patient's disease, but also provide comprehensive attention and guidance to the patient's recovery.

2. New technologies and treatments for breast cancer diagnosis and treatment

Actively carry out standardized diagnosis and treatment of breast cancer, with new technology support provided by the central laboratory and breast pathology department.

The main task is to carry out standardized and individualized treatment of primary breast cancer cases in accordance with the principles of evidence-based medicine, new results in breast cancer research worldwide, and patient needs.

1. Application of new technology in sentinel lymph node biopsy

Currently, lymph node metastasis is an absolute indication for systemic therapy.

In the past, determining whether cancer metastasized to regional lymph nodes mainly relied on axillary lymph node dissection, which may cause upper limb dysfunction. Although it is effective for local treatment, axillary lymph node dissection is an unnecessary over-treatment for breast cancer patients whose lymph nodes have not metastasized.

The application of new minimally invasive lymph node biopsy technology can determine whether cancer metastasis exists in axillary lymph nodes at a small cost, providing an important basis for determining subsequent treatment plans, and can avoid the pain of extended surgery and chemotherapy for patients without lymph node metastasis.

Preliminary research on this technology has been completed and the new technology is ready to enter clinical application.

2. Apply molecular pathology examination results to provide technical support for clinical individualized treatment.

In addition to conventional examination methods to determine the risk of breast cancer, molecular-level indicators have begun to be used to assess risk, and protein-level indicators can guide clinical treatment.

One of the tasks of the Breast Pathology Department is to provide molecular pathology examination results in addition to traditional examination methods, which can predict the efficacy of different programs and the prognosis of patients, and provide technical support for individualized treatment of patients.

3. Neoadjuvant chemotherapy for breast cancer

Locally advanced and recurrent metastatic breast cancer requires effective systemic comprehensive treatment, namely chemoradiotherapy, endocrine therapy and molecular targeted therapy.

Due to the diversity of breast cancer treatments and the uncertainty of individual breast cancer responses to treatment, neoadjuvant systemic therapy is currently a reliable method to ensure the effectiveness of systemic therapy.

The neoadjuvant treatment for breast cancer adopted by our department has become a routine treatment method and is currently in a leading position in the country.

Among the hundreds of cases of neoadjuvant chemotherapy that have been completed, the overall pathological complete response (all tumors disappear) reaches 30%.

4. The effect of breast-conserving surgery for breast cancer has reached international standards

Large-scale clinical studies have confirmed the safety of breast-conserving therapy.

Preserving the breast while ensuring the therapeutic effect can significantly improve the quality of life of breast cancer patients.

With the support of pathological diagnosis and radiotherapy technology, breast-conserving surgical treatment of breast cancer has been widely carried out. The breast-conserving rate is currently leading in the country, and the local recurrence rate is significantly lower than the international average. For breast cancer cases that are not suitable for breast preservation due to large tumors, through effective neoadjuvant treatment, the goal of breast preservation is achieved after shrinking the tumor.

3. Pay attention to the application and promotion of new technologies and treatments to improve the overall treatment level of breast cancer in my country

Since its establishment, while completing medical tasks, it has also focused on scientific research and teaching. It has undertaken and participated in many international and national multi-center clinical trials, many international, domestic and provincial and ministerial scientific research projects, won many provincial and ministerial scientific research achievement awards, and published papers in foreign journals. He has published dozens of articles and academic papers in national magazines. He has participated in the compilation and compilation of many monographs and translations. He has accepted more than 100 students from national oncology training courses to provide comprehensive training in surgery, teaching, scientific research, etc., so that he can become a qualified oncology surgeon.
