Urology Department

Tianjin Medical University General Hospital

Department of Urology, Tianjin Medical University General Hospital: In the development of urology in China, the urology community in Tianjin has always played an important role. Tianjin is also one of the early birthplaces of urology in my country.

During the Anti-Japanese War, due to the Japanese occupation, Peking Union Medical College Hospital was forced to close down. A group of medical experts came to Tianjin. In 1942, Professor Shi Xien, who served as acting director of Peking Union Medical College Hospital, came to Tianjin and founded Enguang Hospital with the famous internal medicine expert Bian Wannian, oncology expert Jin Xianzhai, and obstetrics and gynecology expert Lin Song, and served as the director of the Department of Urology.

Shortly after the victory of the Anti-Japanese War, Professor Yu Songting went to the University of Chicago to study under the internationally renowned urology expert, pioneer of endocrine therapy for prostate cancer, and Nobel Prize winner Professor Charles Huggins.

After Professor Yu Songting returned to China in 1948, he served as the director of the General Surgery Department of Tianjin Central Hospital, one of the five central hospitals of the National Government at that time. He presided over the work of general surgery and gradually established various surgical professional groups, including thoracic surgery, orthopedic surgery, urology surgery, anesthesiology, etc., and later the pediatric surgery department, but still focused on general surgery. Each professional group has a professional attending physician and treats patients within the professional scope. There are about 10 urological surgery beds.

By 1952, in addition to Professor Yu Songting, the urology professional group had grown to include two attending physicians: Ding Houfa and Ma Tengxiang, and a specialized urology clinic was opened.

At this time, the Department of Urology had begun to take shape.

Following this development, by 1972, there were 4 attending physicians and 25 hospital beds.

The members of the Department of Urology are as follows: Department Director: Professor Yu Songting, Deputy Director: Ma Tengxiang, attending physicians: Zhang Zhenxiong, Dong Kequan, Zhang Zuzhao, and Wang Wencheng.

In the 1950s, Professor Yu Songting concurrently served as an assistant professor at Peking Union Medical College, assisting in the reconstruction of the Department of Urology at Peking Union Medical College Hospital, and went to Peking Union Medical College Hospital every week to preside over urology department rounds.

In 1956, Professors Yu Songting and Ma Tengxiang completed the first ileal bladder urinary diversion surgery in China.

Professor Yu Songting also participated in the writing of some chapters.

In the spring of 1973, the original Hebei Provincial Hospital was renamed the Branch of the Affiliated Hospital of Tianjin Medical College, and later renamed the Second Affiliated Hospital of Tianjin Medical College.

According to the government deployment, in order to support the construction of the branch of the General Hospital, Professor Ma Tengxiang led several key doctors to establish the Department of Urology in the hospital.

The main members are Wang Wencheng, Zhang Zuzhao, Dong Kequan, and Yao Qingxiang.

At this time, only Zhang Zhenxiong, the attending physician, remained in the Department of Urology of the General Hospital. Due to the general hospital's patient coverage, popularity, and technical strength, there were still quite a number of patients with urological diseases who came to the hospital for treatment. According to work needs, two doctors, Han Shunan and Shen Mingyu, were transferred to the Department of Urology one after another. In 1976, Dr. Wang Jianyu was transferred, and in 1978, Dr. Qiang Wanming was transferred. There are 18 beds, and they are engaged in the diagnosis and treatment of various diseases in the urology department.

In 1986, the General Surgery Department, which had been followed for many years, was divided into several three-level disciplines. From then on, the Department of Urology worked as an independent specialty. Han Shunan was appointed as the Director of the Department of Urology. At this time, there were 22 beds.

Due to the increase in workload, the original staff could not meet the work needs. Li Liming, Zhang Hui, Ma Hongshun, Liu Guibin, Yang Changhai, Lin Yi, and Cui Zhe were successively transferred to the hospital. By the early 1990s, there were 11 doctors.

Since the early 1980s, Professor Han Shunan was the first to carry out TURP surgery in China. He was one of the first professors to carry out this technology in China.

Director Han Shunan has done a lot of work to promote TURP technology, including giving academic reports at domestic academic conferences, publishing papers and writing monographs, holding study classes, training fellow doctors, and conducting surgical demonstrations across the country. He has made outstanding contributions to the development and promotion of TURP surgical technology.

Professor Han Shunan pioneered transurethral resection of the internal urethral sphincter for the treatment of neurogenic bladder in China, which achieved good results and was included in "Urology" edited by Professor Wu Jieping.

In 1989, Professor Han Shunan served as a member of the Tianjin Urological Society, a member of the Chinese Urological Society, and an editorial board member of the Chinese Journal of Urology. In 1992, he served as the deputy director of the Tianjin Urological Society.

In 1993, Chief Physician Qiang Wanming was appointed as the deputy director of the Department of Urology. In 1998, he was appointed as the director of the Department of Urology and the director of the Surgery Teaching and Research Office of the First Clinical College of Tianjin Medical University.

Dr. Qiang Wanming was the first person in China to perform renal artery angiography to diagnose renovascular hypertension and perform autologous kidney transplantation. By the 1980s, he had performed more than 100 cases of autologous kidney transplantation and achieved good therapeutic results.

In the 1980s, the attending physician Qiang Wanming carried out percutaneous transluminal angioplasty (PTA) to treat renovascular hypertension. This treatment was the first to be carried out in China.

With the strong support of Director Qiang Wanming, the Department of Urology launched laparoscopic minimally invasive surgery earlier in the country.

In 2000, the number of urological surgery beds increased to 32.

There are 16 doctors, including 2 doctors, 1 doctor candidate, 7 masters, 6 senior doctors, and 4 deputy senior doctors.

In 2000, Chief Physician Qiang Wanming served as the deputy director of the Urology Branch of Tianjin Medical Association and an editorial board member of many magazines such as Chinese Urology.

In 2005, the number of urological surgery beds increased to 45.

Professor Li Liming served as deputy director of the Department of Urology in 1999 and director of the Department of Urology in 2006;

He serves as a member of the Urology Branch of the Chinese Medical Association, the deputy director of the Urology Branch of the Tianjin Medical Association, the first and second members of the Urology Branch of the Chinese Medical Doctors Association, the vice president of the Urology Branch of the Tianjin Medical Doctors Association, the deputy chairman of the Urology Professional Committee of the Tianjin Society of Integrated Traditional Chinese and Western Medicine, and a member of the Urology and Male Reproductive System Tumors Professional Committee of the Chinese Anti-Cancer Association;

When the Department of Urology was first established, it was one of the few units in China that established a Department of Urology. At the same time, Professor Yu Songting also helped to reestablish the Department of Urology at Peking Union Medical College Hospital, and was also responsible for teaching. He went to the Department of Urology at Peking Union Medical College Hospital every week to preside over ward rounds.

It has great academic influence in the country.

With the development of socialism with new Chinese characteristics, the business of the Department of Urology of the General Hospital is also gradually developing.

In 1958, a summary of previous medical work was made. Just two figures were given: 1. More than 500 cases of nephrectomy for renal tuberculosis were performed, with no mortality and a very low incidence of complications. Only a few cases of renal ureteral stump fistula were resected.

2. There has been no mortality in more than 200 cases of suprapubic prostatic hyperplasia gland resection.

In 1954, retropubic gland removal for prostatic hyperplasia was performed.

He was the first to carry out the "organ-sparing surgical treatment of renal tuberculosis" in China, and was the first in the country to publish a paper report on "partial nephrectomy" and "lesion removal" for the treatment of renal tuberculosis.

In 1956, Professor Yu Songting completed the first "total cystectomy + ileal bladder surgery (Bricker surgery)" in China. Later, he also carried out "application of intestinal tubes in the field of urology (including colon bladder, bladder enlargement, intestinal replacement of ureter, free intestinal tube to repair epispadias and bladder exstrophy, etc.)" which is also at the leading level in the country.

In 1959, Professor Ma Tengxiang and others succeeded in their research on "Clinical Application of Artificial Kidney", which opened the forefront of domestic blood purification work. Prior to this, peritoneal dialysis was also carried out clinically, which played an active role in rescuing acute renal failure and had a great influence in the country.

In the late 1950s, he was the first to carry out "diagnosis and treatment of renovascular hypertension" in China, and was the first in China to carry out "abdominal aorta-renal arteriography" and "clinical application of isotope renogram" using self-made percutaneous puncture needles.

In 1962, the first "allogeneic kidney transplant" in Tianjin was carried out based on artificial kidney dialysis treatment.

Dialysis treatment for uremia had been playing a leading role in Tianjin at that time.

Since the 1970s, more than 10 cases of allogeneic kidney transplantation have been performed. Due to limitations of immunosuppressants, the postoperative survival rate is not ideal.

In 1972, when ultrasound imaging and CT examination were not yet available, Associate Professor Zhang Zhenxiong relied on his rich knowledge and clinical experience to diagnose and report the first case of reninoma in China.

After that, Zhang Zhenxiong and others successively presided over the diagnosis of renovascular hypertension by renal arteriography, and also carried out renal autotransplantation, achieving good results.

In 1980, Professor Joseph.H.Kaplen of the University of Southern California came to our hospital to teach (transurethral resection of the prostate) TURP and (transurethral resection of bladder tumors) TURBT technology. Since then, Han Shunan began to develop this technology in China. He is one of the first professors to develop this technology in China. Director Han Shunan has performed more than 6,000 TURP surgeries.

Professor Han Shunan has read out papers at many national urological academic conferences and published more than 10 papers in various urology journals. He has trained visiting physicians in dozens of domestic hospitals. Director Han Shunan has also made outstanding contributions to the popularization and promotion of TURP surgical technology by holding study classes and conducting surgical demonstrations across the country.

In addition, Director Han Shunan and others were the first to use cold knife incision to treat urethral stricture in Tianjin and won the Tianjin Science and Technology Achievement Award.

He pioneered the first transurethral resection of internal urethral sphincter to treat neurogenic bladder in China, which achieved good results and was included in "Urology" edited by Professor Wu Jieping.

In 1985, at the Second National Urological Congress, our department published five papers on TURP, TURBT, and PTA.

The work of andrology was developed during this period. Two doctors, Zhang Hui and Yang Changhai, joined the work in this area to diagnose and treat male sexual dysfunction such as ED and premature ejaculation, and worked with obstetricians and gynecologists to carry out assisted reproductive technology work.

Chief Physician Yang Changhai and others also conducted basic clinical research on the relationship between neutrophil elastase and prostatitis, and the relationship between leptin and male sexual dysfunction.

A specialized andrology clinic was opened.

In 2000, urodynamic examination equipment was updated for clinical diagnosis and treatment. Professor Cui Zhe and others conducted urodynamic research on lower urinary tract obstructive diseases, and published several papers in Chinese series of magazines and other core magazines.

In the treatment of urinary system stones, Department of Urology Shen Mingyu, Liu Guibin and others began to carry out extracorporeal shock wave lithotripsy (ESWL) in the 1990s. Since 2000, Liu Guibin, Li Fengkuan and others have successively carried out ureteroscopy (URL) and percutaneous nephroscopy (PCNL).

A specialized clinic for urinary system stones has been opened.

In the early 1990s, the Department of Urology began to pay attention to transurethral laser surgery. Chief physician Qiang Wanming once used Nd-YAG laser to perform vaporization resection of the prostate, but due to the equipment conditions at the time, satisfactory results were not achieved.

After 2005, with the continuous improvement of laser equipment, especially the successful development of some high-power laser equipment, transurethral laser surgery has become a hot spot in the field of urology.

Li Liming and Cui Zhe of the Department of Urology began to use high-power 2um laser equipment to perform vaporization resection of bladder tumors and prostate in 2008. Cui Zhe, Wang Baolong and others tried to use 2um laser to crush larger bladder stones.

Since 2010, Professor Cui Zhe has begun to use thulium laser and plasma equipment to perform transurethral enucleation of the prostate, En-blocTURBt, and achieved good surgical results.

With the popularization of laparoscopic surgery, the Department of Urology, under the leadership of Director Li Liming and Deputy Director Lin Yi, has continuously expanded new surgical indications and carried out laparoscopic nephron-sparing partial nephrectomy, laparoscopic zero-ischaemia nephron-sparing partial nephrectomy, laparoscopic radical prostatectomy, laparoscopic total cystectomy and laparoscopic inguinal lymphadenectomy, single-port laparoscopic surgery, multi-scope combined surgery, etc.

In 2013, Professors Li Liming, Cui Zhe and others performed the first 3D laparoscopic radical prostate cancer surgery in Tianjin.

A 3D laparoscopy technology seminar was held in 2014 to promote this new technology.

In 2016, Professor Li Liming used 3D printing technology for difficult surgeries on complex tumors and achieved good results.

Professors Yu Songting and Ma Tengxiang began research on the application of intestinal tubes in the field of urology as early as the 1950s, and proposed a "modified surgical method of the nipple method for ureteroenteric anastomosis". The clinical application has achieved simple surgery, smooth drainage, anti-reflux and back-pressure effects, and no complications.

He also proposed the "colon bladder" surgical procedure and related research.

Since the early 1980s, Professor Yu Songting and Researcher Wu Mingming have guided graduate students Shi Yongfu, Li Jinsheng, Xue Jian, Ma Hongshun, Li Liming, Lin Yi, Cui Zhe, etc. to carry out a large number of scientific research work focusing on bladder cancer.

Includes: bladder cancer cell surface isotype antigen (ABO antigen);

Chromosome changes in peripheral blood lymphocytes and cancer tissue;

Flow cytometry (FCM) and image analysis (ICM) were used to detect the relationship between DNA content, morphological multi-parameters, pathological grade, clinical stage and prognosis of bladder cancer.

In 1998, Li Liming, Cui Zhe, Wu Mingming and others completed the "Study on the anti-cancer effect of N-CWS on bladder cancer tumor-bearing mice", and the research results won the first prize for scientific research achievements of Tianjin Medical University.

In 2000, Lin Yi, Yu Songting, Wu Mingming and others completed "Research on the Application of Telomerase Activity in Urinary System Tumors" and won the third prize of Tianjin Science and Technology Progress Award.

The research results won the third prize of Tianjin Science and Technology Progress Award.

In 2012, Li Liming and others participated in the clinical multi-center research on "Early Diagnosis and Early Treatment of Prostate Cancer", and the research results won the first prize of Tianjin Science and Technology Progress Award.

In 2009, Yang Changhai, Wang Baolong and others won the third prize of Tianjin Science and Technology Progress Award for their research on the correlation between centriole elastase and prostatitis.

In 2014, Yang Changhai, Zhou Tong, Tang Kunlong and others won the Tianjin Medical University Scientific Research Achievement Award for their research on the correlation between leptin, 5HT and male sexual dysfunction.

In 2015, Tang Kunlong, Wang Liang, Li Jiang, etc. won the third prize of Tianjin Science and Technology Progress Award for their application research on simplified perioperative hormone replacement therapy for patients with hypercortisolism.

As early as 1949, the professional group of urology began to train surgical residents on rotation for half a year.

In 1953, he began to be responsible for the professional lectures and teaching of trainees at Tianjin Medical College, and implemented them according to the teaching syllabus.

After 1956 there was "practical" training for students.

In 1952, it began to recruit regular (one year) and irregular (3 to 6 months) professional training doctors from across the country. By 1972, according to incomplete statistics, it had trained approximately 300 professional doctors. At that time, these doctors were scattered across the country and played a positive role in promoting the development of urology in China.

The training of specialists in this specialty is included in the training plan for surgical residents. After undergraduates graduate, they undergo four years of rotation training in various surgical specialties, then do a half-year residency in the department, and then work as a resident in a professional group.

At various times, in accordance with the teaching plan of Tianjin Medical University, the five-year, eight-year and seven-year clinical teaching work was completed, as well as the English teaching work for international students in the International College.

Since 2010, as a standardized training base for residents, about 100 residents have taken turns to complete training tasks in the Department of Urology every year.

National and provincial continuing medical education training classes and new technology classes are held from time to time.

In 1992, a new technology class for transurethral resection of the prostate (TURP) was held. In 2002, a national urology laparoscopy class was held. In 2004, a urology laparoscopy new technology seminar was held. In 2014, a 3D laparoscopic surgery demonstration and new technology promotion seminar was held.

From 2008 to 2011, deputy chief physician Peng Jie and attending physician Li Jiang went to Xinjiang Hotan County People's Hospital to participate in support work in Xinjiang;

Chief physician Xia Zhenshan went to Dunhuang People's Hospital of Gansu Province to participate in counterpart support work.

In order to cooperate with the development of the hospital-medical alliance, chief physicians Li Liming, Lin Yi, Cui Zhe, Yang Changhai, Zhang Hui, Qiang Wanming and other chief physicians participated in supporting the work of Binhai Hospital. Chief physician Wang Jianyu and other other doctors took turns to participate in supporting community hospitals, airport hospitals, and Airbus medical service tasks.
