Andrology Department

Xiyuan Hospital, Chinese Academy of Chinese Medical Sciences

The Andrology Department of Xiyuan Hospital of the China Academy of Chinese Medical Sciences was established in 1992. It is one of the earliest andrology specialties established in my country. It currently has 3 experts with senior professional titles and 3 doctors with intermediate professional titles, including 1 doctoral supervisor, 1 master supervisor, 1 postdoctoral cooperative supervisor, 1 expert enjoying special government allowances from the State Council, 1 chief researcher of the China Academy of Chinese Medical Sciences, and 1 young and middle-aged famous Chinese medicine practitioner from the China Academy of Chinese Medical Sciences.

At the same time, the Andrology Department of Xiyuan Hospital of the China Academy of Chinese Medical Sciences is an affiliated unit of the Andrology Branch of the China Society of Traditional Chinese Medicine Information and the Andrology Professional Committee of the Beijing Society of Integrated Traditional Chinese and Western Medicine, as well as a clinical, scientific research, and teaching training base.

The department focuses on traditional Chinese medicine and a combination of traditional Chinese and Western medicine, combining overall conditioning with local treatment, with the aim of improving patient safety and efficacy, and highlighting the characteristics of traditional Chinese medicine.

The application of traditional Chinese medicine and integrated traditional Chinese and Western medicine in the treatment of prostatitis, benign prostatic hyperplasia, male infertility, male sexual dysfunction, varicocele, hematospermia, epididymitis, Peyronie's syndrome, genitourinary infections, male menopausal syndrome, male pubertal growth retardation and other male diseases have obvious advantages and characteristics.

In order to facilitate individualized diagnosis and treatment for patients, there are weekly outpatient clinics for male infertility, prostate disease, sexual dysfunction and other special diseases, highlighting the advantages of traditional Chinese medicine.

The department has more than 50,000 outpatient visits per year, ranking among the top three men's departments in Beijing. Patients come from all over the country and European and American countries, and it has a certain influence at home and abroad.

1. Clinical

(1) Service and treatment philosophy: Focus on people-oriented, humanized services, and individualized treatment.

(1) Respect the rights and interests of patients and pay attention to protecting patient privacy;

(2) Adopt the andrology diagnosis and treatment model of first-diagnosis responsibility and full-process management;

(3) Adopt a treatment plan that combines overall conditioning with local conditioning;

(4) The treatment concept is oriented towards safety and efficacy, using models such as acupuncture combined with medicine and acupuncture and cupping to highlight the characteristics of traditional Chinese medicine;

(2) Academic philosophy:

Based on the ancient prescriptions of traditional Chinese medicine combined with the summary of clinical experience, an academic concept with the characteristics of the Andrology Department of Xiyuan Hospital has been formed:

(1) Propose the "Liver, Spleen, and Kidney Theory": The diagnosis and treatment of andrological diseases are based on the liver, spleen, and kidneys. The kidneys, liver, and spleen are the basis of male physiological functions. If the metabolism of the kidneys, liver, and spleen is normal, the kidney essence is sufficient and the Tiangui is abundant;

Liver blood is strong and catharsis is moderate;

Good temper, good luck, and abundant Qi and blood.

The pathogenesis of andropathies is based on deficiency of qi in the liver, spleen, and kidneys, and dampness and heat, phlegm, blood stasis, and qi stagnation are the symptoms.

A variety of effective prescriptions have been created clinically based on the "Liver, Spleen and Kidney Theory".

(2) Emphasis on "treating from the liver" andrological diseases: With the progress of society and the acceleration of the pace of life, modern people's material living standards have improved. Excessive intake of fat, sweet and thick flavors has led to overnutrition, and the disease has become younger. People's physiques have also changed. The nature of the disease is more evidence-based. There are fewer male problems caused by true kidney deficiency. Especially some disease symptoms in men under 60 years old are mostly related to the liver.

(3) Promote the "prevention before disease occurs" theory: pay attention to men's sub-health status.

The traditional Chinese medicine saying "don't treat the disease, treat the disease before it's gone" illustrates the importance of preventing the disease before it's discovered. For andrological diseases such as prostate disease, male infertility, and sexual dysfunction, focusing on regulating the patient's sub-health status will have the effect of getting twice the result with half the effort.

Based on this, the department created an alternative tea drink to improve men's sub-health status - Phyllanthus emblica and wolfberry drink (composed of: Phyllanthus emblica, wolfberry, etc.).

(1) Scope of diagnosis and treatment

Specialized in the diagnosis and treatment of traditional Chinese medicine and integrated traditional Chinese and Western medicine:

① Prostate diseases (acute and chronic prostatitis, benign prostatic hyperplasia, prostate cancer);

②Male infertility (oligoasthenoteratozoospermia, analgesic semen, azoospermia, microtesticles, male factors for fetal infertility, traditional Chinese medicine conditioning before assisted reproductive technology, etc.);

③Sexual dysfunction (low sexual desire, erectile dysfunction, premature ejaculation, inability to ejaculate, etc.);

④ Urinary tract infection (urethritis, mycoplasma, chlamydial infection, gonorrhea, non-gonococcal urethritis, etc.);

⑤ Delayed-onset hypogonadism (male menopausal syndrome);

⑥ Other male diseases, such as epididymitis, hematospermia, balanitis, varicocele, developmental delay in children and other genitourinary system diseases.

For more than 20 years, the department has been continuously exploring the diagnosis and treatment of male diseases with traditional Chinese medicine and integrated traditional Chinese and Western medicine. It is based on the syndrome differentiation and treatment theory of traditional Chinese medicine and combined with international scales such as IIEF-5, NIH-CPSI, IPSS, and PEDT.

The purpose is to improve patient efficacy and improve patients' quality of life.

The modern instrument testing equipment has advanced sperm quality testing systems and urinary flow rate testers (imported). In terms of male sexual function diseases, it has carried out domestic and foreign advanced testing methods such as multi-parameter hardness testing of the penis, penis sensitivity test, vasoactive drug injection test in the corpus cavernosum + color Doppler ultrasonography of the penis, providing conditions for the clear diagnosis of male diseases.

Treatment uses drugs (traditional Chinese medicine, integrated traditional Chinese and Western medicine), psychological consultation, acupuncture, cupping, heat therapy and other methods, which are very popular among patients.

(2) Characteristics of andrology

1. Prostatitis.

Emphasizing that stagnation of dampness and heat is the basic pathogenesis of prostatitis, a series of Qianlie Huazhuo prescriptions were developed by combining oral decoction and external acupuncture during the treatment: Qianlie Huazhuo No. 1 prescription (Phellodendron, Centella asiatica, red peony root, fried Atractylodes, etc.).

Qianlie Huazhuo No. 2 prescription (Yam, Sanleng, Rhodiola rosea, etc.);

Qianlie Huazhuo No. 3 prescription (peach kernel, safflower, adenophora, forsythia, etc.);

Qianlie Huazhuo No. 4 prescription (lychee core, orange core, fennel, etc.);

Qianlie Huazhuo No. 5 prescription (danggui, cassia twig, white peony root, dried ginger, etc.) and other series of traditional Chinese medicines.

2. Benign prostatic hyperplasia.

For mild to moderate benign prostatic hyperplasia, Xiyuan Andrology has developed a series of prescriptions for Qiangli Tongqiang:

Qianli Tongguan Recipe No. 1 (composed of Cortex Cortex, Atractylodes, Xu Changqing, fried barley, etc.);

Qianlie Tongguan Recipe No. 2 (composition: Prunella vulgaris, Fritillaria fritillaris, Platycodon grandiflorum, almond, etc.);

Qianlie Tongguan Prescription No. 3 (composed of: cassia twig, poria, peach kernel, red peony root, etc.);

Qianlie Tongguan Prescription No. 4 (composed of: Wuyao, Yizhiren, Astragalus, Guizhi, etc.) advocates "tonifying the kidneys, activating blood and replenishing the lungs" to treat benign prostatic hyperplasia combined with weak detrusor contraction. It can significantly improve the clinical symptoms of prostatic hyperplasia, reduce urinary retention and reduce the chance of surgery. Some prostates have a tendency to shrink in size;

On December 29, 2010, the 52nd issue of "Chinese Medicine", the four international channels of CCTV, aired the program "Getting the disease down and getting it up smoothly" by Director Guo Jun of the Andrology Department of Xiyuan Hospital for the treatment of prostate hyperplasia.

Combining disease and syndrome, further summarizing the evolution of TCM syndromes of prostatic hyperplasia and the rules of TCM and Western medicine diagnosis and treatment, and standardizing the clinical treatment principles and corresponding prescriptions of traditional TCM, it is expected to provide more effective comprehensive treatment for patients, especially patients with prostatic hyperplasia accompanied by weak detrusor contraction of the bladder, resulting in long-term indwelling urinary catheter.

Over the years, the Men's Department of Xiyuan Hospital of the China Academy of Chinese Medical Sciences has used acupuncture, cupping, and traditional Chinese medicine to nourish the kidneys, activate blood, and nourish the lungs, which has significantly improved the success rate of urinary extraction, freed patients from the pain of long-term indwelling urinary catheters, improved patients' symptoms of dysuria, and improved their quality of life.

3. Male infertility.

The Andrology Department of Xiyuan Hospital is the team leader for the compilation of the "Guidelines for the Diagnosis and Treatment of Integrated Traditional Chinese and Western Medicine in Male Infertility" by the Andrology Professional Committee of the Chinese Society of Integrated Traditional Chinese and Western Medicine. It is responsible for the dominant disease of the Chinese Academy of Chinese Medical Sciences - male infertility, and is the first to conduct clinical research on it in our hospital.

Features of traditional Chinese medicine include

(1) Taking the kidneys as the center and taking into account the liver and spleen: the agreed prescriptions are Liuwu Shengjing Decoction (composed of: Rehmannia glutinosa, Cuscuta semen, wolfberry, raspberry, angelica, etc.), Qixiong Seed Decoction (composed of: Astragalus, Chuanxiong, Rehmannia glutinosa, Millet Spatholobus, etc.) ), Bushen Tiaogan Shengjing Decoction (composed of: Rehmannia glutinosa, Bupleurum, Cuscuta, Plantain Seed, etc.), Linggui Prescription (Angelica Sinensis, Xianlingpi, Rehmannia glutinosa, Lycium barbarum, etc.), and were funded by the China Academy of Chinese Medical Sciences and the National Natural Foundation of China.

(2) It is advocated that "microscopic semen differentiation" is used to treat low sperm density, nourishing yin and kidney, nourishing blood and replenishing essence;

Sperm motility is not enough to nourish the kidneys and strengthen yang. The method of replenishing qi and activating blood circulation can increase sperm motility;

For those with poor semen liquefaction, methods of clearing heat, detoxifying, promoting qi, and promoting blood circulation should be used to promote semen liquefaction.

Male infertility after failed in vitro fertilization is treated with the method of nourishing the kidneys and soothing the liver and creates Chaihuang Zanyu Formula (composed of: Bupleurum, Rehmannia glutinosa, raw malt, fried Atractylodes, etc.).

(3) Propose the theory of sperm regulation: The theory of sperm regulation includes treating sperm, nourishing sperm, and strengthening sperm, with sperm as the axis and conception as the common purpose.

① To treat semen with abnormal semen parameters and obvious clinical symptoms, a series of prescriptions have been created including: Ziyin Tianjing Decoction (composed of turtle shell, anemarrhena, dogwood, poria, etc.);

Huanggui sealing marrow soup (composition: Rehmannia glutinosa, turtle shell glue, deer antler glue, Codonopsis pilosula, etc.);

Guben Jianyang Decoction (composed of: Deer Antler Glue, Cistanche deserticola, Rehmannia glutinosa, Amomum villosum, Angelica sinensis, etc.);

Liquefied soup (composition: cork, barley, barley, plantain, etc.)

② Nourishing semen means that after treatment in the semen treatment stage, the sperm indicators of the patient return to normal and there are no clinical symptoms, but the woman is still not pregnant. During this stage, while continuing to take drugs, attention is paid to life conditioning to prevent semen parameters from decreasing. Related prescriptions have been created including: Lujia Erxian Decoction (composed of: deer antler glue, tortoise shell, mulberry, dendrobium, etc.);

Longmu Jiaotai Decoction (composed of: dragon bone, coptis, jujube kernel, Eclipta chinensis, Bupleurum, etc.), etc.

③ If the sperm parameters of the strong sperm are normal but the clinical symptoms are present, they are in a sub-health state. Create a relevant prescription: Xiaoyao Shunqi Decoction (composed of: Bupleurum, costus, white peony root, Citrus aurantium, etc.);

Qifu Yiyang Decoction (composition: Astragalus, Cyperus Cyperus, Heishunpian, dried ginger, etc.).

For more than 20 years, the scientific research team headed by Director Guo Jun has developed a series of prescriptions and combined with the in-hospital preparation of the impotence-replenishing oral liquid, which can effectively improve the sperm quantity, quality and motility of male infertility patients, especially severe oligoasthenozoospermia, microtesticles, semen liquefaction, etc., and at the same time improve the fertilization rate of assisted reproductive technology;

Among them, Director Guo Jun’s Linggui prescription has remarkable clinical efficacy in treating asthenozoospermia, and was funded by the National Natural Science Foundation of China in 2014 for further research;

Qixiongfang was funded by the National Natural Science Foundation of China Youth Science Fund in 2017.

In addition, a series of examinations such as pre-childbearing consultation and pre-childbearing physical examination are carried out.

4. Erectile dysfunction (ED).

In terms of diagnosis: The department has the American Rigiscan penis hardness tester, which is currently recognized as a relatively objective method for identifying psychological ED and organic ED. It can comprehensively monitor the erectile activity of the penis, and can also dynamically monitor the number of penile erections, erection duration, erection hardness, swelling, etc., and has many advantages such as no trauma and no pain.

Penile Doppler blood flow detection is available in the color ultrasound room in the hospital. Color Doppler blood flow scintigraphy is used to observe the changes in penile arterial and venous hemodynamics during penile erection, which provides reliable objective indicators for the diagnosis of arteriovenous and venous ED. It is one of the most valuable examination methods for clinical differential diagnosis of ED and identifies the cause of erectile dysfunction.

Treatment: Kidney-tonifying, liver-soothing and blood-activating formula series No. 1 formula for nourishing the kidney, soothing the liver and activating blood circulation (composition: rhodiola rosea, turmeric, centipede, acanthopanax, etc.);

Recipe No. 2 for nourishing the kidney, soothing the liver and activating blood circulation (composed of epimedium, Morinda officinalis, Cynomorium cynomorium, Acanthopanax senticosus, Salvia miltiorrhiza, etc.);

Recipe No. 3 for nourishing the kidneys, soothing the liver and activating blood circulation (composition: antler cream, tortoise shell glue, turmeric, salvia, etc.).

It treats functional erectile dysfunction to nourish the kidneys, activate blood, strengthen the body, and restore sexual function. It advocates the concept of "treating the mind and body at the same time, integrating the whole body (body health) with the local part (penis), emphasizing that only with good health can sexual function be good and sexual life can be harmonious." This concept has been recognized by patients.

Comprehensive intervention such as the integration of traditional Chinese and Western medicine can avoid or reduce the side effects of simple Western medicine intervention to a certain extent and improve the clinical efficacy.

5. Premature ejaculation.

The department has a human body sensory threshold testing and analysis system for auxiliary examination of premature ejaculation.

Traditional Chinese medicine believes that the deficiency of kidney qi and the inability to absorb semen are the root cause of premature ejaculation. Long-term illness and stagnation of liver qi leads to the loss of ejaculation. Hypericum perforatum is used as the king medicine to form the department-agreed prescription Qiaoshao prescription (composed of hypericum perforatum, white peony root, calamus, calcined keel, calcined oyster, etc.). It has remarkable clinical efficacy in treating premature ejaculation. It was funded by the National Natural Science Foundation of China in 2012 and 2018 for further research.

The kidneys govern the storage of essence. Deficiency of kidney yin and failure of the essence connection can also cause premature ejaculation. Based on this, the protocol Ziyin Gujing Decoction (composed of: Rehmannia glutinosa, Polygonatum, Chinese yam, dendrobium, Ziziphus seed, etc.) was created, which has a good effect in treating premature ejaculation.

In addition to traditional syndrome differentiation, based on the theory of "the principle of external treatment is the principle of internal treatment, the medicine of external treatment is the medicine of internal treatment, and the difference is the law", Wuding Delay Spray, an external medicine for premature ejaculation, was developed and achieved good clinical effects.

6. Varicocele.

Mild or moderate varicocele can be treated conservatively with traditional Chinese medicine, and the Quyu Huashi Decoction (composed of: Salvia miltiorrhiza, Achyranthes bidentata, Cortex Phellodendri, Fried Atractylodes, etc.) has been created;

If the effect of conservative treatment is not obvious, surgical treatment can be used.

For severe varicocele that affects semen quality, it is recommended to undergo surgical treatment first. After surgery, it can be combined with Guiqi Quyu Decoction (composed of: angelica, astragalus, red peony root, cinnamon twig, black medicine, etc.) to improve local blood supply after surgery and improve sperm quality.

Traditional Chinese medicine has unique advantages in treating varicocele. Under the guidance of the theory of syndrome differentiation and treatment of traditional Chinese medicine, methods such as clearing away heat and dampness, promoting blood circulation and removing blood stasis are advocated to treat varicocele, which shortens the patient's recovery time, improves the patient's quality of life, and significantly improves the patient's prognosis.

7. Epididymitis.

Acute epididymitis is divided into early, middle and late stages with different treatments: in the early stage, it clears away heat and detoxifies, in the middle stage, it clears heat, detoxifies and activates blood circulation and removes blood stasis, and in the late stage, it focuses on softening and dispersing stagnation.

The epididymis decoction developed by the department (composed of: Prunella vulgaris, honeysuckle, Panax notoginseng, Fritillaria fritillary, etc.) has the effects of clearing away heat and detoxification, activating blood circulation and removing blood stasis, softening and dispersing stagnation, and has good curative effect on shrinking the swollen epididymis and alleviating the pain and other discomforts of patients.

The development of epididymis decoction to treat chronic epididymitis can significantly improve pain and local blood circulation in the epididymis.

8. Hematospermia.

We launched an investigation into the cause of hematospermia, and used a series of self-developed traditional Chinese medicine hematospermia prescriptions: Xuejing No. 1 Prescription (composed of: Astragalus, Bletilla, Daji, Xiaoji, etc.);

Xue Jing No. 2 Recipe (composed of: Xue Yutan, Panax notoginseng, Salvia miltiorrhiza, calcined oyster, etc.);

Xue Jing No. 3 prescription (composed of: Astragalus, Puhuangtan, safflower, Atractylodes, etc.) can treat hematospermia (seminal vesiculitis) with an effective rate of 80% and a treatment course of 3 months.

9. Functional anejaculation.

The clinical symptoms of functional anejaculation are often divided into four types: liver stagnation and qi stagnation, kidney essence deficiency, damp-heat stasis, and phlegm-dampness stagnation for syndrome differentiation and treatment. In the treatment, special emphasis is placed on unblocking the collaterals throughout the treatment. In the later stages of each syndrome type, kidney-tonifying drugs are added to improve gonadal function. In each syndrome type, the application of liver-soothing methods must be taken into consideration, and psychological counseling should be combined with drug treatment;

The prescription for soothing the liver, nourishing the kidneys and unblocking the collaterals (composed of epimedium, passepartout, luffa, Trachelospermia, etc.) developed by the department can treat functional anejaculation with an effective rate of 90% and a course of treatment of 1-2 months.

10. Prostate cancer.

The Qianlie Fuzheng prescription (composed of: Astragalus, Codonopsis, Angelica, Hedyotis, Shanzigu, etc.) comprehensively treats prostate cancer. It can increase the immune function of postoperative and advanced prostate cancer patients, improve the postoperative quality of life of prostate cancer patients, prolong the patient's survival time, delay cancer progression, and prevent cancer recurrence.

11. Foreskin/Phimosis.

For conservative treatment, external Chinese medicines such as heat-clearing, detoxifying, diuretic and itching are used, and external washing prescriptions are created (composed of: Sophora flavescens, Cortex Phellodendron, White Fresh Bark, Kochia Semen and other medicines).

Sleeve surgery can be used to treat prepuce and phimosis, which has the advantages of neatness and beauty after surgery;

The disposable circumcision stapler/stapler used in surgery is simple and fast and is very popular among patients.

12. Peyronie’s disease.

It refers to the fibrotic lesions of the tunica albuginea of the corpus cavernosum of the penis, causing single or several plaques or induration to appear on the back or outside of the penis.

This disease is more common in adults. Plaques can cause penile erection pain and curvature deformity, which can lead to difficulty in sexual intercourse.

Under the guidance of TCM syndrome differentiation, the department used the method of promoting blood circulation and removing blood stasis to create the Sanjie Qiao Decoction (composed of: raw oysters, Scrophulariaceae, Fritillaria fritillaris, Panax notoginseng powder, Verbena, etc.) to gradually eliminate penile induration and reduce the patient's pain. If necessary, local injections of verapamil and other drugs are given for treatment. The effect is accurate and the course of treatment is 1-2 months.

13. Sexually transmitted diseases.

We have extensive experience in the treatment of sexually transmitted diseases such as gonorrhea, mycoplasma, chlamydia infection and genital warts. Along with traditional antibiotic treatment, we also use oral or topical application of traditional Chinese medicine such as heat-clearing, detoxifying and antiviral medicines, thereby improving the patient's immunity and preventing the recurrence of sexually transmitted diseases.

2. Scientific research
