Department introduction: The Department of Respiratory and Critical Care Medicine (PCCM Department) is a supporting unit of the National Center for Respiratory Medicine, the National Clinical Research Center for Respiratory Diseases, the Institute of Respiratory Medicine of the Chinese Academy of Medical Sciences, and the Department of Respiratory Medicine of Capital Medical University. It is also a national key discipline (Respiratory Diseases in Internal Medicine), a national key clinical specialty, and a demonstration unit for the standardized construction of PCCM. The department has begun to develop its own characteristics in the fields of diagnosis and treatment of lung cancer, respiratory intervention and sleep disordered breathing. 1. Respiratory failure, critical illness in internal medicine, and respiratory support technology have a complete respiratory and critical care medical system that conforms to the modern respiratory disease pattern, and have established a systematic and complete organ monitoring and support technology system: The system has established a standardized respiratory support technology system and is the largest extracorporeal membrane oxygenation (ECMO) rescue center for respiratory failure in my country. Treatment center; the system has established standardized respiratory and circulatory monitoring technology; diagnosis and treatment of various common respiratory critical illness, especially in the field of diagnosis and treatment of special respiratory tract infection critical illness; treatment of sepsis, various types of shock and multiple organ failure (MODS); weaning and rehabilitation of ventilator-dependent patients. 2. COPD In recent years, a lot of achievements have been made in the clinical diagnosis and treatment of COPD, respiratory rehabilitation, new drug development and clinical research. He has presided over and participated in a number of international and domestic multi-center clinical studies of new COPD drugs, and is currently conducting a stem cell therapy study and multiple post-registration clinical studies.At the same time, it created a standardized assessment and intervention plan for COPD respiratory rehabilitation, and developed a series of COPD rehabilitation training tutorials and classes, which were recognized by single-course students. 3. Asthma and Chronic Cough has achieved domestic leading results in the treatment of severe (refractory) asthma. It was the first to carry out bronchial thermoplasty in China and is the unit that has completed the most surgical cases in Asian countries. It took the lead in formulating the bronchial thermoplasty operation and perioperative management specifications in my country. A systematic asthma phenotype assessment method was established and personalized precision treatment was carried out. Cough sensitivity and induced sputum cytology tests were carried out, and leading results were achieved in the diagnosis and treatment of cough. 4. Respiratory System Infection We are the first in China to establish a diagnosis and treatment model that bundles microbiology laboratories with respiratory infections, develop and utilize the most advanced international pathogen diagnostic technology, establish the most complete bacterial and fungal culture and drug sensitivity, antigen, and nucleic acid diagnostic systems in the country, and cooperate with the clinical pharmacy team to truly achieve precise treatment under the guidance of respiratory infection pathogens. Every year, more than 2,000 difficult infection cases are transferred from various places and treated. 5. Pulmonary embolism, pulmonary hypertension, pulmonary heart diseaseUnder the leadership of Academician Wang Chen and Director Zhai Zhenguo, we have long been engaged in the diagnosis and treatment of pulmonary vascular diseases such as pulmonary embolism and pulmonary hypertension. We organize multi-disciplinary groups such as the Department of Respiratory and Critical Care Medicine, Cardiac Surgery, Lung Transplantation, Ultrasound Diagnosis, Radiation Diagnosis, Nuclear Medicine, and Rheumatology and Immunology to treat pulmonary embolism, chronic thromboembolic pulmonary hypertension, other types of pulmonary hypertension, pulmonary vasculitis and other pulmonary vascular diseases. Pulmonary vascular interventional procedures such as pulmonary thromboendarterectomy, pulmonary artery balloon dilatation, pulmonary artery stent, biopsy of intrapulmonary artery lesions, bronchial artery embolization, right heart catheterization, and pulmonary angiography are routinely performed. 6. Interstitial lung diseaseAt present, the ILD professional group has a multidisciplinary expert team with Professor Dai Huaping as the subject leader, and respiratory specialists, respiratory imaging, respiratory pathology, thoracic surgeons, rheumatology and immunologists and other clinical and basic respiratory researchers as the backbone. It has established a relatively standardized system for the diagnosis, treatment and follow-up of interstitial lung disease and patients. The data management system relies on the "National 13th Five-Year Plan Key Research and Development Plan - Precision Medicine Special Project - Respiratory Disease Specialized Cohort Study - Interstitial Lung Disease Cohort Study" to carry out long-term enrollment and follow-up of patients with interstitial lung disease, which has greatly promoted the improvement of clinical diagnosis and treatment of ILD and the establishment of a standardized research system for ILD. 7. The annual lung transplant operation volume is about 100, ranking among the top three in the country. The department has rich experience in the diagnosis, treatment and transplantation evaluation of patients with end-stage lung disease, lung transplant surgery, perioperative multidisciplinary comprehensive management, pulmonary rehabilitation guidance, and long-term postoperative follow-up of patients. It can complete complex lung transplant operations including difficult lobe transplantation, severe pulmonary hypertension lung transplantation, concurrent coronary artery bypass lung transplantation, and combined liver-lung transplantation. There are 2,000 bronchoscopy cases per year, including more than 200 level IV airway interventional surgeries per year. 8. Respiratory rehabilitationA multidisciplinary team led by PCCM doctors and composed of rehabilitation doctors, physical therapists, respiratory therapists, swallowing therapists, PCCM department nurses and other business backbones has been established to carry out a multidisciplinary respiratory rehabilitation ward round consultation system, build an acute exacerbation period - stable period - home respiratory rehabilitation management system based on Internet of Things standards, and integrate patient file management, respiratory rehabilitation assessment, intervention and prevention. It has accumulated rich experience in respiratory rehabilitation time and formed professional diagnosis and treatment features with leading advantages. It took the lead in carrying out early respiratory rehabilitation intervention in critical illness in my country and established a standardized process for ultra-early respiratory rehabilitation in ICU; it systematically established standardized postoperative management and early rehabilitation for lung transplant patients, providing strong support and guarantee for the perioperative survival rate of lung transplantation; it established a national respiratory rehabilitation collaboration network to assist the national chronic disease prevention and treatment project; it established a standardized diagnosis and treatment system for respiratory rehabilitation and became a demonstration training center for respiratory rehabilitation diagnosis and treatment in my country. 9. Lung cancer Relying on the National Respiratory Center, the lung cancer professional team focuses on the diagnosis and treatment of lung nodules, early diagnosis and screening of lung cancer and minimally invasive treatment, precise targeted/immunotherapy for lung cancer, multidisciplinary comprehensive treatment, lung cancer comorbidities and treatment-related adverse reactions.The lung cancer MDT platform has been established in conjunction with the radiotherapy department, thoracic surgery department, pathology department, imaging department, and interventional department to provide "one-stop multidisciplinary expert team" diagnosis and treatment services for patients with difficult and complex lung cancer. 10. The respiratory interventional diagnosis and treatment area of ​​the PCCM Department of Respiratory Intervention was established in 2017 under the guidance of Academician Wang Chen, and has been gradually developed and improved. It is currently the training base for respiratory endoscopists in my country. The respiratory interventional diagnosis and treatment team has carried out a series of clinical and applied basic research in the field of interventional diagnosis and treatment technology for chronic airway diseases, lung cancer, interstitial lung disease, lung transplantation and pleural disease, and has begun to take shape; it has developed respiratory interventional diagnosis and treatment technology based on flexible bronchoscopy, rigid bronchoscopy, medical thoracoscopy, and navigation bronchoscopy (electromagnetic navigation system, optical positioning navigation system), and is gradually establishing a transvascular, transairway, and percutaneous multi-channel interventional respiratory diagnosis and treatment technology system and training system. 11. Sleep breathing disorderThe sleep respiratory clinic has 8 sleep monitoring and treatment units that meet international standards, and carries out different levels of sleep monitoring, multiple nap latency tests, non-invasive carbon dioxide monitoring, gastroesophageal reflux monitoring, nasopharyngeal acoustic reflex, artificial pressure titration, cognitive behavioral therapy for insomnia and other projects. It is a comprehensive sleep disorder diagnosis and treatment center for the diagnosis and treatment of sleep disorders, insomnia, sleep behavior disorders, biological rhythm disorders, narcolepsy, restless legs syndrome and other sleep diseases.
