After subtotal gastrectomy, the remnant stomach is anastomosed to the jejunum, and the space left behind the anastomosis is called the retroanastomotic space. Its edges lack elasticity, resembling a hernia ring. If an intestinal loop protrudes into the retroanastomotic space and cannot reduce naturally, an internal hernia after gastrectomy forms. Internal hernia after gastrectomy is clinically rare, most commonly seen after Billroth II subtotal gastrectomy and gastrojejunostomy. It can occur in the early or late postoperative period, with the most common site being the lower posterior space formed after retrocolic gastrojejunostomy.
