Post-gastrectomy bile reflux gastritis is caused by the reflux of bile, pancreatic juice, and other fluids into the gastric remnant, irritating the mucosa and resulting in gastritis. The incidence is higher after Billroth II surgery than after Billroth I surgery. Reflux gastritis is primarily caused by the mixing of bile and intestinal fluid, which refluxes through the pylorus into the stomach, thereby irritating the gastric mucosa and producing inflammation. The main causes of reflux gastritis include prior subtotal gastrectomy with gastrojejunostomy, functional pyloric disorders, and chronic biliary tract diseases. If left untreated over time, it may progress to gastric ulcer, gastric perforation, or even gastric cancer.
