Tropical spastic paraparesis (TSP) is an endemic neurological disease prevalent in many tropical and subtropical countries. It is a spinal cord inflammation caused by chronic infection with human T-lymphotropic virus type 1 (HTLV-1). The disease has an insidious onset and slow progression, with spastic paraparesis as the main clinical manifestation. Anti-HTLV-1 antibodies are positive in blood and cerebrospinal fluid. Glucocorticoids have some therapeutic effect.
